Objective: To summarize and compare the improvement degree of cardiorespiratory function,lower limb function and activity of daily living of stroke patients before and after different intensity of power bicycle training for stroke patients,so as to analyze and compare the difference between two different training methods,and try to find the best intensity for cardiorespiratory rehabilitation training for stroke patients,To provide theoretical basis for the cardio-pulmonary rehabilitation and overall rehabilitation methods and programs of stroke patients.Methods: This study was a randomized controlled study.From January 2022 to February 2023,32 inpatients with stroke who met the inclusion criteria of the experiment were selected from Beijing Xiaotangshan Hospital,28 of whom were finally completed the study,and 4 were exfoliated(2 in the high-intensity group and 2in the anaerobic threshold group).The patients were randomly divided into two groups,high intensity group and anaerobic threshold group,with 14 subjects in each group.The baseline data of the three groups of subjects are consistent and comparable.The subjects in both groups received routine rehabilitation and power cycling training.Routine rehabilitation training is the same in the two groups.Routine rehabilitation training is arranged according to the actual situation of the patient,including manual therapy(PNF technology,Bobath technology,etc.),respiratory muscle training,gait training,balance training,center of gravity transfer training,good limb positioning,daily living activity training,etc.Rehabilitation training is required once a day and five times a week;The training program of power cycling training in the anaerobic threshold group and the high intensity group is the same.The difference is that the training intensity of the two groups is different.The training intensity of the anaerobic threshold group is anaerobic threshold intensity,and the training intensity of the high intensity group is anaerobic threshold + 30%(35)intensity((35)=peak power-anaerobic threshold power).Once a day,five times a week,and four weeks of intervention.Cardiopulmonary exercise tests were conducted before and after the rehabilitation training intervention,and the test and intervention process were carried out under the strict supervision of the tester.The test and training environment has the ability to deal with adverse events in an emergency manner,which can ensure the safety of patients.By collecting and comparing the cardiopulmonary function indicators(peak oxygen uptake,relative peak oxygen uptake,anaerobic threshold,maximum load,oxygen pulse,etc.)of patients before and after the intervention Static lung function index(tidal volume,one-second volume,one-second rate of maximum voluntary ventilation,etc.),lower limb function index(Fugl-Meyer Assessment of Lower Extremity)and activity of daily living index(Modified Barthel Index).The data were analyzed by SPSS20.0 statistical software.Levels of statistic significant were set as P< 0.05.Observe,compare and analyze the improvement degree of related indicators of stroke patients after rehabilitation intervention and the difference between the two groups.Result:(1)Cardiopulmonary function indicators: There were significant differences in maximum oxygen uptake,relative maximum oxygen uptake,and anaerobic threshold between the two groups before and after treatment(P<0.05);The treatment effect of the high intensity group was better than that of the anaerobic threshold group,and the difference between the two groups was statistically significant(P<0.05).(2)Cardiopulmonary indexes of exercise: There were significant differences in minute ventilation volume,maximum heart rate,oxygen pulse,and maximum power between the two groups before and after treatment(P<0.05);The maximum heart rate and maximum power in the high intensity group were significantly different between the two groups after intervention(P<0.05).(3)Static pulmonary function indicators: After 4 weeks of intervention,the forced vital capacity,slow vital capacity,one second rate,maximum voluntary ventilation volume,and tidal volume in the anaerobic threshold group were statistically significant compared to the indicators before the intervention(P<0.05),There was also a statistically significant difference between the static pulmonary function indicators in the high intensity group and those before training(P<0.05),Comparison of forced vital capacity,one second rate,and tidal volume between the two groups showed no statistically significant differences after intervention(P>0.05),while the differences in slow vital capacity and maximum voluntary ventilation were statistically significant(P<0.05).The effect of the high intensity group was better.(4)Lower limb function indicators: After the intervention,the scores of the simplified lower limb Fugl-Meyer evaluation scale of the two groups of subjects had a statistically significant difference compared to those before the intervention(P<0.05),and after the intervention,the effect of the high intensity group was better than that of the two groups(P<0.05).(5)Activity of daily living indicators: After training,the scores of the modified Barthel index between the two groups were significantly different from those before training(P<0.05),and after training,the effect of the high intensity group was better than that of the two groups(P<0.05).Conclusion:(1)Four weeks of power cycling training can effectively improve the cardiopulmonary function,lower limb function,and daily activity ability of patients after stroke.(2)In terms of cardiopulmonary function,4-week high intensity intermittent training can more effectively improve the cardiopulmonary function of patients after stroke compared to anaerobic threshold intensity,and the training process is safe and effective.(3)In terms of rehabilitation of lower limb function and improvement of activities of daily living in patients with stroke,both anaerobic threshold intensity and high intensity intermittent training can improve the functional status of stroke patients,but the effect of high intensity rehabilitation is better than that of anaerobic threshold intensity training.
