| Research purpose:1.To clarify the original meaning of Zhongjing’s texts about blood impediment disease, return to Zhongjing’s clinical diagnosis and treatment decision-making mode of "Disease-PulseSyndrome combined Treatment",and discuss each part of the whole procedure "differentiating the disease→identifying the pulse → analyzing the syndrome→determining the treatment"concerning blood impediment disease.2.To analyze the formulated principle of Huangqi Guizhi Wuwu Decoction and sort out the evolution of this prescription in order to provide guidance for the present clinical application.Research method:Chapter Ⅰ:Research the original meaning of Zhongjing’s texts about blood impediment disease. The methods are as follows.1.Edition collation:The study mainly referred to Deng Zhen’s collated version of Synopsis of the Golden Chamber.Further edition collation was conducted referring to Yu Qiao,Xu Rong,Zhao Kaimei,Wu Qian’s edition,as well as other medical books such as Mai Jing,The etiology and symptoms of various diseases, and Beiji Qianjin Yaofang, among which Wu Qian’s edition published in the Hongwu period of Ming Dynasty, was the key reference. For those with significant differences, their correctness verified in order to restore their original appearance to the maximum extent possible.2.Word meaning and sentence punctuating research:Word meaning and sentence punctuating of blood impediment disease related texts were revised based on the relevant language materials in Qin and Han dynasties and the language habits of Zhongjing’s period.3.Analysis of difficult problems:Analysis of the main difficult problems existing in blood impediment disease related texts was carried out according to the results of word meaning reaserch, and later medical scholars’ comments on the texts of Zhongjing was sorted out in order to present the original meaning.Chapter Ⅱ:Reserrch on blood impediment disease based on the Zhongjing’s clinical diagnosis and treatment decision-making mode of "Disease-Pulse-Syndrome combined Treatment".The specific methods are as follows.1.Literature research:Later medical scholars’ comments on each part of the procedure"differentiating the disease→identifying the pulse→analyzing the syndrome→determining the treatment" concerning blood impediment disease were sorted out to show the true and comprehensive understanding of blood impediment disease.2.Sorting and analyzing medical records:With the help of digital tools such as Duxiu,Chinese Medical Code and CNKI,medical records related to Huangqi Guizhi Wuwu Decoction was systematically collect, sorted and analyzed.Research results:The mean results of Chapter I are as follows.1.In the first clause of blood impediment disease,"zun rong people" should be "zun le people" according to Wu Qian’s edition, which means a man of great rank and comfort."Zun rong people" is very likely to be miswritten by the passer.2.According to Zhong Jing’s language habits, the first clause of pulse of blood impediment disease should be read as "The pulse is slightly astringent, small tight in the cunkou and guanshang".3."Like wind impediment" should be "like the wind" according to Wu Qian’s edition, which is a supplementary description of "body numbness".It describes that the body is painless and not itchy, like the wind blowing. The "wind" is used as a verb.4.According to Wu Qian’s edition,Platycodon Grandiflorum Decoction can also be used to treat "throat impediment" rather than "blood impediment".The note in small size here may be annotated according to other medical literature when the Song ministers collated the book.Deng Zhen wrote that Platycodon Grandiflorum Decoction "can also treat blood impediment".It may be due to the wrong information in his references,or because he made a mistake himself.5.The "sheng" in "gu ruo ji fu sheng" refers to obesity."Ji fu sheng" means people have a plump physique and their striaes are loose.6."Chong yin" should be pronounced as "chóng yīn",which means "also because"."Bu shi" is a negative construction, meaning "inappropriate"."Dong yao" refers to physical activity, indicating that "zun le people" should do a series of activities themselves, such as sex,dao yin,and ordinary work.If these activities take place when people should have a break,it is inappropriate.7."Yin and Yang are both insufficient" in the second clause of pulse of blood impediment disease should refer to the deficiency of Yin and Yang,Qi and blood,Ying and wei and pulse. The mean results of Chapter II are as follows.1.The name of blood impediment disease should be seen for the first time in the unearthed medical inscribed wooden slips of Laoguanshan Han Tomb, which is named according to the symptoms that blood circulation is obstructed.Later generation named it with the character"wind" because it is usually caused by wind.2.The internal factor of the cause of blood impediment disease is body deficiency."Evil"refers to all external factors, such as wind, cold,or dampness, and wind-cold,etc.These "evils"are the incentives of blood impediment disease.3.As for the disease position,The Yellow Emperor’s Classic of Internal Medicine only mentions that "evil enters into Yin".Blood belongs to Yin aspect, and it circulates in the vessels. When affected by evil, blood vessels will be obstructed. Therefore, the disease position should be in blood vessels.4.The main pulse of blood impediment disease is "slight and astringent",that is,slight pulse and astringent pulse are seen together.Slight pulse refers to the weakness of qi and blood, while astringent pulse refers to the astringency of the pulse with unsmooth flow of blood. This is the pathological basis of blood impediment disease.5."Body bu ren, like the wind" is the main syndrome of blood impediment disease. Its clinical manifestations are body numbness,no sensation of pain and itchy. At the late stage of the disease, when qi and blood stagnation is serious, patient will feel pain.During the course of the disease, there will be some other symptoms, such as sweating, fear of wind, fatigue and so on.The further transmission of blood impediment disease may lead to collapse, gangrene and other symptoms.6.The treatment of blood impediment disease is divided into acupuncture therapy and decoction therapy. As for acupuncture therapy, acupuncture site is not fixed, but the shallow needling method should be used and he accupoint of Yang meridians should be selected. As for decoction therapy,Huangqi Guizhi Wuwu Decoction is the main prescription. This prescription has been well inherited and applied in the later generations, and the indications are constantly expanding. At the same time, later generations has developed this prescription. For example, modern interpretation of blood impediment disease appeared in late Qing Dynasty and the early Republic of China. It is believed that disease is related to peripheral nerve damage,so the prescription is always used in the treatment of nervous system diseases and limbs and skin diseases, greatly expanding the application scope of the prescription.Research conclusion and significance:1.This study researched the original meaning of blood impediment disease from the aspects of editions collation and word meaning research, and obtained a part of the understanding that is not completely the same with the existing understanding. These understandings are based on reliable methods and documentary evidence, and to a certain extent, they clarify the original meaning of Zhong Jing’s original meaning.2.In the perspective of "Disease-Pulse-Syndrome combined Treatment",blood impediment disease has been comprehensively sorted out and discussed.Blood impediment disease is induced by exogenous evils on the basis of Yang weakness due to fatigue consuming energy.Evil qi obstructs the blood,so the blood is not smooth, and Ying-wei is blocked, which causes the symptoms such as body numbness and no sensation of pain and itchy.Zhongjing propose acupuncture therapy and decoction therapy in the treatment of blood impediment disease, both of which reflect the meaning of "Tongyang and Xingbi".3.In modern clinic, the category of blood impediment disease and theapplication scope of Huangqi Guizhi Wuwu Decoction are expanding continuously, mainly covering the diseases of immune system and nervous system.Later doctors have applied the treatment in many miscellaneous diseases such as Raynaud’s syndrome, diabetic peripheral neuropathy, etc. At the same time,Huangqi Guizhi Wuwu Decoction is also used in the treatment of a variety of joint diseases, like rheumatoid arthritis, cervical spondylosis, scapulohumeral periarthritis with limb pain or numbness. |
