Purpose:By data mining and analyzing as well as by sorting the academic thinking and clinical experience,this study is to elucidate Professor Wang Qingguo’s clinical diagnosis and treatment model and prescription medication patterns for climacteric syndrome,and the diagnosis and treatment plan for Liver stagnation and Spleen deficiency syndrome of climacteric syndrome is proposed to highlight Professor Wang Qingguo’s theoretical and clinical innovation in differentiation and treatment of climacteric syndrome with Liver stagnation and Spleen deficiency syndrome,and new ideas and methods for improving the clinical diagnosis and treatment level of climacteric syndrome is provided.Methods:1.Research of data mining on Professor Wang Qingguo’s diagnosis and treatment of climacteric syndromeMedical records of Professor Wang Qingguo’s outpatient treatment of climacteric syndrome from October 2013 to October 2022 were collected,and 474 patients who meet the diagnosis of climacteric syndrome,being treated with totaling of 921 prescriptions,were ultimately included.After Standardizing the treatment of symptoms,the prescriptions,and other information in Professor Wang Qingguo’s medical records,we input them into the Ancient and Modern Medical Record Cloud Platform to establish a database.By further utilizing data mining methods such as frequency statistical analysis,cluster analysis,and complex network analysis in the platform,data analysis was conducted based on the general information,relevant surgical history,menstrual abnormalities,first-time four diagnosis data,and medication status of 474 patients,in order to organize the objective basis for dialectical diagnosis and explore the internal connections and modifying rules of treatment formulas.2.Summary of Professor Wang Qingguo’s academic thought and clinical experience in diagnosis and treatment of climacteric syndromeBy literature reading,learning from teachers,course lectures,and dialogue interviews,this article summarizes Professor Wang Qingguo’s academic ideas and main experiences in diagnosis and treatment of climacteric syndrome under the disease-syndrome diagnosis and treatment model,and analyzes Professor Wang Qingguo’s theoretical basis for differentiation and treatment of climacteric syndrome with Liver stagnation and Spleen deficiency.3.Diagnosis and treatment plan of climacteric syndrome with Liver depression and Spleen deficiencyBased on Professor Wang Qingguo’s academic ideas,clinical experience,and data mining analysis in treatment of climacteric syndrome,this paper summarizes the common clinical manifestations and medication characteristics of climacteric syndrome with Liver stagnation and Spleen deficiency,and elaborates the diagnostic criteria and treatment plans in treating climacteric syndrome with Liver stagnation and Spleen deficiency.Results:1.Research on data mining of Professor Wang Qingguo’s diagnosis and treatment of climacteric syndromeA total of 474 female patients,aged 40-60,with climacteric syndrome were included in the study,with an average age of 51 years.Among them 30 cases(6.3%)have undergone menopausal related surgery and 248 cases with abnormal menstrual conditions reported,accounting for 52%.The common symptoms of them include insomnia,rush Heat,excessive sweating,irritability,menstrual disorders,palpitations,fatigue,menopause,shallow sleep,fear of cold,difficulty in falling asleep,dry mouth,gastric distension,tinnitus,cold lower limbs,loose stools,short breath,and bitter mouth.Cluster analysis was conducted on the top 48 symptoms of frequency,and 9 groups were obtained:①excessive sweating,rush Heat,insomnia,menstrual disorders,shallow sleep,dreaminess,difficulty in falling asleep,chest tightness,short breath and palpitations;②Anxiety and tension,irritability,headache,dizziness,fatigue,high blood pressure;③Peripheral pain,joint pain,hot feet,hot palms;④Low menstrual flow,constipation,and flank pain;⑤Dry eyes,tinnitus,bitter mouth,dry mouth,dry stools,indigestion,cold limbs;⑥Loose stools,menopause,night sweats;⑦Acid reflux,epigastric pain,hiccup,gastric distension,sticky stools,cold stomach,abdominal distension;⑧Fear of wind,fear of cold,cold lower limbs,hysterectomy,spontaneous sweating;⑨Back pain,low back pain,amenorrhea.With "Symptoms-Symptoms" complex network analysis,28 nodes were obtained:excessive sweating,rush Heat,insomnia,irritability,menstrual disorders,palpitations,shallow sleep,difficulty in falling asleep,fatigue,menopause,dry mouth,anxiety and tension,dreaminess,fear of cold,hot palms,hot feet,short breath,tinnitus,gastric distension,dizziness,loose stools,chest tightness,cold lower limbs,cold limbs,bitter mouth,dry eyes,headache,and fear of wind,which are the core symptoms of climacteric syndrome.The tongue condition with the highest frequency is characterized by a pale tongue;The tongue coating with the highest frequency is manifested as greasy coating and white coating;The pulse with the highest frequency is manifested as slippery pulse.The high frequency of Chinese medicines used are Chaihu(Bupleurum),Zhigancao(Processed licorice),Huangqin(Scutellaria baicalensis),Fabanxia(Pinellia sinensis),Duanmuli(Calcined oyster),Fuxiaomai(Floating wheat).Danggui(Angelica),Guizhi(Cassia twig),Dazao(Jujube),Huangqi(Astragalus).Fuling(Poria cocos),Dangshen(Codonopsis pilosula),Zhifuzi(Processed radix aconiti lateralis),Duanlonggu(Processed fossilized bone),Huangbai(Golden cypress),Shoudi(Prepared rehmannia root),Chaosuanzaoren(Fried sour jujube kernel),Chaobaizhu(Fried white atractylodes rhizome),Xianlingpi(Herba epimedii),Tusizi(Semen cuscutae),etc.The drug properties are mainly warm,slightly cold,flat,cold and slightly warm,and their distribution is fairly balanced.The main medicinal tastes are sweet,hot,bitter and sour.The main related meridians are Spleen,Liver meridians and so on.Cluster analysis was made on the top 50 Chinese herbs with the highest frequency,and 10 core combinations were extracted as follows:①Xixin(Asarum sieboldin mig),Zhimahuang(Processed Chinese ephedra),Zhifuzi(Processed radix aconiti lateralis),Fangfeng(Saponikovia cata)and Chaobaizhu(Fried white atractylodes rhizome);②Huangqi(Astragalus),Danggui(Angelica),Dangshen(Codonopsis pilosula),Roucongrong(Cistanche),Tusizi(Semen cuscutae)and Shoudi(Prepared rehmannia root);③Mohanlian(Eclipta),Nǚzhenzi(Fructus lingustri lucidi),Sangye(Folium mori);④ Huangqin(Scutellaria baicalensis),Chaihu(Bupleurum),Fabanxia(Pinellia sinensis),Duanmuli(Calcined oyste)r,Baishao(Radix paeoniae alba),Zhigancao(Roasted licorice),Guizhi(Cassia twig),Dazao(Jujube),Fuxiaomai(Floating wheat),Duanlonggu(Processed fossilized bone),Chaosuanzaoren(Fried sour jujube kernel),Renshen(Ginseng),Yejiaoteng(Tuber fleeceflower stem),Xiakucao(Prunella spike);⑤Danpi(Root bark of peony),Zexie(Rhizoma alismatis),Chaozhizi(Fried gardenia),Chaoshanyao(Fried yam),Shanyurou(Common macrocarpium fruit);⑥Shengdi(rehmannia root)and Huangbai(Golden cypress);⑦Maidong(Radix ophiopogonis),Wuweizi(Schisandra chinensis),Baiziren(Semen platycladi),Xianhecao(Agrimonia pilosa),Xianlingpi(Herba epimedii);⑧Qianghuo(Notopterygium root),Niuxi(Radix achyranthis bidentatae),Gegen(Root of kudzu vine),Chuanxiong(Ligusticum wallichii);⑨Chantui(Cicada slough),Lianqiao(Fructus forsythiae);⑩ Chenpi(Pericarpium citri reticulatae),Fuling(Poria cocos),Ganjiang(Dried ginger)and Huanglian(Coptis chinensis).Eleven nodes were obtained from the complex network analysis of "Chinese medicinal-Chinese medicinal",namely,Chaihu(Bupleurum),Zhigancao(Roasted licorice),Baishao(Paeoniae),Huangqin(Scutellaria),Fabanxia(Pinellia sinensis).Fuxiaomai(Floating wheat),Duanmuli(Calcined oyster),Dazao(Jujube),Guizhi(Cassia twig),Danggui(Angelica)and Huangqi(Astragalus),which are the core prescription drugs used for Professor Wang Qingguo’s treatment of climacteric syndrome.2.Summary of Professor Wang Qingguo’s academic thought and clinical experience in diagnosis and treatment of climacteric syndromeProfessor Wang Qingguo believes that climacteric syndrome originates from Kidney deficiency,with symptoms such as imbalance of Yin and Yang as well as the restlessness of the Zang and Fu organs;The main syndrome can bea divided into two categories,namely,the Yin kind and the Yang kind,which refers to Liver and Kidney Yin deficiency as well as Liver and Spleen imbalance;It is advocated that the treatment combination of disease and syndrome,widely using of classic prescriptions,reasonable combining of prescriptions,and applying the main prescription for the main disease should be achieved in the whole process;It is also proposed to promote the peace and harmony between Yin and Yang as well as between Zang and Fu organs,in which regulating the central cardinal Qi to harmonize Liver and Spleen,to harmonize Qi and blood,to harmonize Ying Qi and Wei Qi,and to harmonize Yin and Yang.In clinical treatment of climacteric syndrome,modified Chaihu Guizhi Decoction are often used with Longhu Ganmai Dazao Decoction,Zishui Qinggan Decoction are often used with Longhu Ganmai Dazao Decoction.For patients with climacteric syndrome who have newly developed metrorrhagia,metabolic syndrome,and rheumatic immune disease,treatment should be based on the severity of the varied conditions of the syndrome.3.Study on diagnosis and treatment plan of climacteric syndrome with Liver depression and Spleen deficiencyThe key points of syndrome differentiation are as follows:cardinal dysfunction of Shaoyang as well as Spleen and Stomach Qi,obstruction of Qi and blood,disharmony between Ying Qi and Wei Qi,coexistence of Cold and Heat syndrome.The main symptoms include menstrual disorders,coexistence of rush Heat induced sweating and fear of cold,insomnia and restlessness;secondary symptoms include dry throat,bitter mouth,muscle and joint pain along Taiyang and Shaoyang meridians,abdominal distension,and loose stools.The treatment policy is soothing the Liver and strengthening the Spleen,relieving depression and regulating the cardinal Qi,harmonizing Ying Qi and Wei Qi as well as coordinating Yin and Yang.The main prescription is Chaigui Guiqi Ganmai Dazao Decoction:Chaihu(Bupleurum)10g,Huangqin(Scutellaria baicalensis)10g,Fabanxia(Pinellia sinensis)10g,Guizhi(Cassia twig)10g,Baishao(Radix paeoniae alba)30g,Danggui(Angelica)15g,Huangqi(Astragalus)30g,Duanmuli(Calcined oyster)30g,Zhigancao(Roasted licorice)30g,Fuxiaomai(Floating wheat)30g,and Dazao(Jujube)20g.According to the example of drug addition and subtraction,the prescription can be modified flexibly.Conclusion:1.By analyzing the syndrome differentiation basis and prescription patterns of Professor Wang Qingguo in treatment of climacteric syndrome,it is found that Professor Wang Qingguo can flexibly formulate prescriptions according to different concurrent symptoms,based on differentiation of Yin and Yang case in treatment of climacteric syndrome,and his innovated medication further improve the clinical efficacy in treatment of the disease.2.By analyzing and summarizing Professor Wang Qingguo’s academic thinking and clinical experience in differentiation and treatment of climacteric syndrome.It is found that climacteric syndrome is caused by Kidney Deficiency,with Liver and Kidney Yin deficiency induced Yang hyperactivity and cardinal Qi dysfunction of Liver and Spleen as the main pathogenesis.In diagnosis and treatment,great attention should be paid to regulating the cardinal Qi to restore the balance between Yin and Yang in respects of Ying Qi,Wei Qi and blood performance.3.Based on Professor Wang Qingguo’s experience,a diagnosis and treatment plan in treatment of climacteric syndrome with Liver stagnation and Spleen deficiency is proposed.This common syndrome type in current clinical practice of climacteric syndrome should be given greater attention;The use of the classic formula Chaihu Guizhi Decoction combined with Ganmai Dazao Decoction in treatment has expanded the application scope of the classic formula Chaihu Guizhi Decoction,which is of great significance in promoting effective treatment formulas and improving the life quality of climacteric syndrome patients.4.Professor Wang Qingguo’s unique experience in using Chaihu Guizhi Decoction and Ganmai Dazao Decoction to treat climacteric syndrome with Liver stagnation and Spleen deficiency is of great clinical and academic value,providing a clinical basis for development of new drugs for climacteric syndrome.It also provides new ideas for improvement of traditional Chinese medicine diagnosis and treatment standards as well as innovation of theoretical system of differentiation and treatment for climacteric syndrome. |
