| Objective:To explore the correlation between the Constitutional types of Chinese medicine and anxiety and depression state in colorectal adenoma patients by investigating the gender,age,body mass index and other general conditions of colorectal adenomatous polyp patients,the Constitutional types of Chinese medicine and anxiety and depression state.From the perspective of psychosomatic treatment,this paper aims to provide a reference for the clinical prevention and treatment of colorectal adenomatous polyps.Methods:The cases in this study were from 186 patients with colorectal polyps who met the inclusion and exclusion criteria who were pathologically diagnosed as adenomatous polyps in the outpatient clinic and ward of the Department of Proctology of Guanganmen Hospital,China Academy of Chinese Medical Sciences from January 2022 to January 2023.By filling in the case report form,the patient’s general condition,Constitutional types of Chinese medicine,anxiety and depression state and colonoscopy data were collected,and an Excel database was established for statistical analysis using SPSS 20.0.Results:1 This study finally included 186 patients with colorectal adenomatous polyps,101 males and 85 females,with a male-to-female sex ratio of 1.19:1,an age range of 28～79 years,an average age of 59.11±10.02 years,8 cases in the youth group(18～44 years old),40 cases in the middle-aged group(45～59 years old),and 55 cases in the elderly group(≥60 years old).2 Colonoscopy of 186 patients with colorectal adenomatous polyps:161 cases of polyps were first found,and 25 cases of recurrence;The number of polyps was 49 cases and 137 cases.There were 26 cases of polyp size<0.5 cm,111 cases of 0.5～1 cm,and 49 cases of>1 cm.There were 70 cases of polyps in single sites and 116 cases in multiple parts.Among the pathological types,there were 152 cases of tubular adenoma,21 cases of tubular villous adenoma,3 cases of villous adenoma and 10 cases of serrated adenoma.3 The difference in polyp size and pathological type distribution of 186 patients with colorectal adenomatous polyp was statistically significant(χ2=12.103,P=<0.05),tubular adenoma and serrated adenoma were most common in the 0.5～1.0 cm group,tubular villous adenoma and villous adenoma were more common in the>1.cm group,while no villous adenoma was seen in the<0.5 cm group.4 Distribution of Constitutional types of Chinese medicine among 186 patients with colorectal adenomatous polyps:65 cases(34.9%)of Gentleness type,121 cases(65.1%)of pathological constitution.The pathological constitution was 38 cases of Wetheat type(20.4%)>36 cases of Yang-deficiency type(19.4%)>16 cases of Qidepression type(8.6%)>9 cases of Qi-deficiency type(4.8%)>8 cases of Phlegmwetness type(4.3%)>7 cases of Yin-deficiency type(3.8%)>4 cases of Bloodstasis type(2.2%)>3 cases of Special diathesis type(1.6%).5 There were significant differences in the distribution of Constitutional types of Chinese medicine among 186 patients with colorectal adenomatous polyp(P<0.05).Men are more likely to see Wet-heat type and Yang-deficiency type,and women are more likely to see Yang-deficiency type and Qi-depression type;Smokers are more likely to have Wet-heat type,and smokers are denied more likely to have Yang-deficiency type;Exercise ≤1 time/week is more common Wet-heat type,2～3 times/week is more likely to see Wet-heat type and Yangdeficiency type,≥4 times/week is more likely to see Yang-deficiency type;Sedentary people are more likely to see Wet-heat type,and those who deny sitting are more likely to see Yangdeficiency type.6 Among the 186 patients with colorectal adenomatous polyps,115 patients(61.8%)had no anxiety state and 71 patients(38.2%)had anxiety state,including 48 cases of mild anxiety state,16 cases of moderate anxiety state,and 7 cases of severe anxiety state;There were 138 cases(74.2%)without depression state and 48 cases(25.8%)with depression state,including 31 cases of mild depression state,9 cases of moderate depression state,7 cases of moderate and severe depression state,and 1 case of severe depression state.7 There were significant differences between gender,smoking history,hypertension and anxiety state in 186 patients with colorectal adenomatous polyps(P<0.05).The incidence of anxiety state was higher in women,those who denied smoking and those with hypertension.There were significant differences between gender,smoking history,drinking history,staying up late and depression state(P<0.05).Women,those who denied smoking,denied drinking and often stayed up late had a higher incidence of depression state.8 There were no significant differences between the pathological type and Constitutional types of Chinese medicine,whether anxiety state,depression state,and degree of anxiety state and depression state in 186 patients with colorectal adenomatous polyps(P>0.05)9 There was significant difference between Constitutional types of Chinese medicine and anxiety state and depression state in patients with colorectal adenomatous polyp(P<0.001).In the state of anxiety,there were significant differences between Gentleness type and Qi-deficiency type,Wet-heat type,Blood-stasis type,Qi-depression type and Yangdeficiency type and Qi-depression type(P<0.0016).In the state of depression,there was significant difference between Gentleness type and Qi-deficiency type,Wet-heat type,Bloodstasis type and Qi-depression type(P<0.0016).There was no significant difference in the degree of anxiety state and depression state(P>0.05).From the frequency of anxiety state and depression state in different physiques,people with anxiety state and depression state are more common in Wet-heat type,Qi-depression type and ang-deficiency type,while people without anxiety state and depression state are more common in Gentleness type.Conclusions:1 In this study,the main pathological types of colorectal adenomatous polyps were tubular adenomas,followed by tubular villous adenomas and serrated adenomas,and villous adenomas were the least common.The size of polyp is related to the distribution of pathological type.the larger the polyp is,the worse the differentiation of pathological type is.2 In this study,colorectal adenomatous polyps are more common in Constitutional types of Chinese medicine,among which Wet-heat type is the most common,followed by Yangdeficiency type,Qi-depression type,Qi-deficiency type,and Phlegm-wetness type.3 Patients with colorectal adenomatous polyps are prone to psychological problems such as anxiety state and depression state,and various degrees of anxiety state and depression state may occur.4 There was no correlation between the pathological type of colorectal adenomatous polyp and Constitutional types of Chinese medicine,the existence of anxiety,depression state and the degree of anxiety and depression state.5 The Constitutional types of Chinese medicine of colorectal adenomatous polyps has a certain correlation with anxiety state and depression state,and those with pathological constitution are more likely to have anxiety state and depression state. |
