Objective:To evaluate the clinical effectiveness of Professor Liu Daxin’s empirical formula "Tongqiaojingxuan Decoction "in the treatment of residual symptoms of primary benign paroxysmal positional vertigo(BPPV)after repositioning of otoliths with liverdepression and spleen-deficiency,further confirming that the treatment of otoliths with the liver and spleen as the entry point has extensive theoretical and literature support,which is conducive to the development of drug-formulation studies and the optimization of treatment protocol for residual dizziness after otolithic repositioning.Methods:Eighty cases with residual symptoms of primary posterior semicircular canal BPPV(PC-BPPV)were included and divided into treatment and control groups,and baseline levels between the two groups were counted.In the treatment group,granules of Tongqiaojingxuan Decoction were used,and in the control group,no-decoction Chinese medicine placebo granules were used.The results of mCTSIB examination in both groups before and after treatment were recorded,and the DHI scale,SAS anxiety self-assessment scale,SDS depression self-assessment scale and vertigo Chinese medicine evidence scale were filled in for statistical analysis.Results:1.There was no statistically significant difference between the two groups before treatment in terms of gender,age,side involved,insomnia,underlying disease and duration of vertigo before repositioning(P>0.05).The results of mCTSIB examination,total and sub-item values of DHI scale,SAS anxiety scale score,SDS depression scale score and vertigo Chinese medicine symptom scale score were not statistically significant before treatment in both groups.2.mCTSIB test results:In the mCTSIB test results of the two groups,Foam-EO and Foam-EC decreased after treatment compared with the pre-treatment results,which was statistically significant(P<0.05)and the reduction of Foam-EO and Foam-EC results in the treatment group was better than that in the control group after treatment(P<0.05).3.Dizziness Disability Inventory(DHI):the DHI total score,DHI-P(physical),DHI-E(emotional)and DHIF(functional)scores decreased significantly in both groups after treatment(P<0.001),and the DHI total score,DHI-P and DHI-E values decreased to a greater extent after treatment than in the control group(P<0.001).4.Self-assessment Scale for Anxiety(SAS)and Self-assessment Scale for Depression(SDS):After treatment,SAS and SDS scores decreased in both groups compared with those before treatment(P<0.05),and SAS and SDS scores decreased to a greater extent in the treatment group than in the control group(P<0.05).5.Clinical symptom evaluation scale for vertigo:the TCM clinical symptom score decreased in both groups after treatment compared with that before treatment(P<0.001),and the TCM clinical symptom score decreased to a greater extent in the treatment group than that in the control group(P<0.05),especially in improving the symptoms of "dizziness,lightheadedness,headache,insomnia,sleeplessness and tiredness"(P<0.05).6.After a treatment course of 14 days,8 cases were cured,12 cases showed efficacy,13 cases were effective,and 3 cases were ineffective,with a total effective rate of 91.67%;in the control group,2 cases were cured,8 cases showed efficacy,18 cases were effective,and 10 cases were ineffective after a treatment course of 14 days,with a total effective rate of 73.68%(P<0.05).Conclusion:Tongqiaojingxuan Decoction can effectively improve the subjective dizziness symptoms after successful PC-BPPV repositioning,shorten the duration of residual symptoms,improve the TCM clinical symptoms of residual dizziness in PC-BPPV patients with liver-depression and spleen-deficiency,and relieve anxiety and depression.In conclusion,Tongqiaojingxuan Decoction has remarkable efficacy and is worth promoting in clinical practice.
