| Objective: to observe the changes of serum albumin level of hip and knee joints in elderly patients three days before surgery,and analyze the risk factors of hypoproteinemia induced changes in cardiac function,so as to provide diagnostic basis for clinical workMethods: a total of 64 elderly patients who underwent hip or knee replacement in our hospital from december 2021 to december 2022 were included in this retrospective study by screening and exclusion criteria.serum ALBumin content of the patients within three days after surgery was examined.based on the average serum protein(alb)after surgery,the patients were divided into normal group(alb > 25g/l)and hypoproteinemia group(alb < 25g/l).The following cardiac function index levels were also monitored on the third postoperative day by the NICAS impedance cardiac output monitoring system: heart rate,diastolic blood pressure,systolic blood pressure,cardiac stroke volume,stroke index,cardiac output,cardiac index,cardiac power index,left ventricular systolic function,total peripheral resistance,total peripheral resistance index,cardiac reserve,variation per stroke,Single factor,multiple factor and correlation analysis were performed using Graph Pad Prism 9.0 software.When the P value was less than 0.05,the results showed statistically significant differences.Finally,the relevant risk factor indicators of hypoproteinemia-induced cardiac function changes were obtained through summary and analysis.Result1.Finally,86 elderly patients who underwent hip joint replacement or knee joint replacement in our hospital were selected into this study,including 21 males and 65 females,aged 60–92 years old,with the average age of 75 3.21 years old,37 patients who underwent hip joint replacement and 49 patients who underwent knee joint replacement.Serum albumin content was checked in all 86 patients before surgery.The results showed that the serum albumin content in our patient was as high as 55g/L and as low as 27 g/L.According to the diagnostic criteria for hypoproteinemia,the number of patients with hypoproteinemia after hip replacement was 12,and the number of patients with hypoproteinemia after knee replacement was 18.2.The probability of hypoproteinemia occurring three days after operation is 34.8%,among which the probability of hypoproteinemia occurring after hip joint replacement is 32.4%,and the probability of hypoproteinemia occurring after knee joint replacement is 36.7%.The results of independent sample t-test analysis showed that there were significant changes in cardiac stroke volume,cardiac output,cardiac power index,left ventricular systolic function,and stroke volume variation index in the normal group and the hypoproteinemia group three days after surgery(P < 0.05).3.The linear correlation analysis showed that the indexes of stroke volume,cardiac output,cardiac power index,left ventricular systolic function,stroke volume variation,cardiac index,and cardiac reserve had correlation with the decrease of serum albumin content(P < 0.05).The most relevant index was stroke volume SV(P=0.013,r=0.921).4.A two-category Logistic analysis was performed on the previously related cardiac function indicators,and the results showed that there were significant differences in cardiac stroke,cardiac output,cardiac power index,left ventricular systolic function,variation of stroke volume,cardiac index,and cardiac reserve(P < 0.05). |
