Purpose:Based on syndrome differentiation system,the common TCM disease locations and distribution characteristics of syndrome elements in type 2 diabetic peripheral neuropathy complicated with depression were analyzed,and the correlation between syndrome elements and general data and physical and chemical indicators were explored to facilitate clinical work.Method:In this study,96 patients with type 2 diabetic peripheral neuropathy with depression who were admitted to the Department of Endocrinology affiliated Hospital of Liaoning University of Traditional Chinese Medicine from December 2020 to December 2021 were enrolled to evaluate their depression status by Using the Zung Self-rating Depression Scale(SDS).The patients’basic information,TCM four diagnosis information,physical and chemical indicators and other data were collected,and TCM syndrome element were diagnosed based on syndrome differentiation.The distribution of TCM syndrome elements was analyzed by statistical software SPSS26.0,and the correlation between syndrome elements and general information and physical and chemical indicators was explored.Results:1.There were 96 subjects in this study,including 51 males and 45 females in terms of gender.In age,age.There were 15 cases in young people aged 45 years,accounting for 15.6%,and 33 cases in middle-aged people aged 45-60 years,accounting for34.4%.There were 48 patients aged 60 years,accounting for 50.0%.Middle-aged and elderly patients were the majority of patients included in this study,accounting for84.4%.In terms of course of disease,54 patients were less than 10 years old,accounting for 56.3%,34 patients were 10-20 years old,accounting for 35.4%,and 8patients were more than 20 years old,accounting for 8.3%.Most of the patients had a course of disease less than 10 years,most of them were middle-aged people aged45-60 years old,and most of them were more than 20 years old.Sixty year old people;In the degree of depression,56 patients with mild depression accounted for 58.3%,35patients with moderate depression accounted for 36.5%,and 5 patients with severe depression accounted for 5.2%.The longer the course of depression,the lower the proportion of patients.Patients with mild depression accounted for the largest proportion in the three course groups,and patients with severe depression were mostly distributed in the course of disease less than 10 years.2.In this study,there were a total of 693 syndrome elements in 96 patients with type 2diabetic peripheral neuropathy complicated with depression.The frequency distribution of disease location syndrome elements was highest in liver(85 cases,accounting for 88.5%),followed by mind(61 cases,63.5%),spleen(56 cases,58.3%),kidney(56 cases,58.3%),heart(18 cases,18.8%),stomach(17 cases,17.7%),lung(11 cases,11.5%),channels and collaterals(10 cases,10.4%),the rest of the composition ratio of less than 10%is half surface and half li.The proportions of liver,mind,spleen and kidney were all over 50%.Qi-stagnation was the highest in 64 cases,accounting for 66.7%,followed by blood stasis(61 cases,63.5%),Yin deficiency(60cases,62.5%),Yang deficiency(32 cases,33.3%),qi-deficiency(30 cases,31.3%),phlegm(25 cases,26.0%),cold(25 cases,26.0%),dampness(20 cases,20.8%),fever(15 cases,15.6%),blood deficiency(14 cases,14.6%),and the other components below 10%were jin deficiency,Yang hyperactivity,dryness and blood fever.Among the disease syndrome elements,the proportion of qi stagnation,blood stasis and Yin deficiency all exceeded 50%.There were differences between the symptoms of dampness,Yin deficiency,phlegm,heat and BMI without diagnosis(P<0.05);There were differences in depression scores between patients with liver,mind,qi stagnation,cold,blood stasis and those without diagnosis of the syndrome(P<0.05);Qi stagnation,blood stasis and Yin deficiency were the most distributed in HBA1C 7-9%respectively.There was a difference in conduction velocity between blood stasis and right common peroneal nerve and right tibial nerve without diagnosis of the syndrome(P<0.05).Conclusion:1 The main syndrome elements of type 2 diabetic peripheral neuropathy complicated with depression were liver,mind,spleen and kidney.The main syndrome elements are qi stagnation,blood stasis,Yin deficiency,Yang deficiency and qi deficiency.2 In the depression score of patients with type 2 diabetic peripheral neuropathy complicated with depression,the higher the depression score,the more prone to occurrence of disease location syndrome element of liver,mind,disease syndrome element of qi stagnation,blood stasis,cold;In terms of body mass index,patients with higher BMI are more likely to have Yin deficiency,phlegm,dampness and heat.The frequency of qi stagnation,blood stasis and Yin deficiency were more distributed in Hb A1c.In terms of nerve conduction velocity,the worse the nerve conduction function,the easier it is to diagnose blood stasis.3 Qi stagnation and blood stasis syndrome elements are the key to the pathogenesis of type 2 diabetic peripheral neuropathy complicated with depression.
