Purpose:1.The luminex multifactor immuno quantitative detection and screening technology was used to analyze the change of cytokine expression level in purpura nephritis,and to study whether it was involved in the pathogenesis of HSPN and the target of Chinese medicine intervention through cytokine expression level.2.By comparing the cytokine expression levels of children with hot poisoning blood stasis purpura nephritis before and after intervention in traditional Chinese medicine(thistle kidney decoction)with healthy children,the efficacy mechanism and target of traditional Chinese medicine intervention in HSPN were studied whether the corresponding cytokine level changes were studied.Material and method:1.Sample collection and grouping: In the outpatient clinic of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine,the purpura kidney group(HSPN)that meets the inclusion criteria was selected,and healthy children were recruited as the healthy group(H).Inform the patients and parents in detail of the purpose of the research project they are participating in,and the guardian and the person themselves sign the consent form. Venous blood samples from the healthy group,the HSPN group,and after 1 course(4 weeks)of traditional Chinese medicine treatment were collected.This research protocol was approved by the Medical Ethics Committee of Liaoning University of Traditional Chinese Medicine.2.The luminex multifactor immuno quantitative detection technology is used to detect the quantitative horizontal expression of cytokines,including MCP-1,G-CSF,TNF-α,IL-2,IL-5,IL-6,IL-7 and IL-10.The baseline data of the study subjects was analyzed using SPSS26.0 software,and the counting data was expressed as a rate or composition ratio,using chi-square tests,and the measurement data were expressed as mean ± standard deviation(X±S).In line with the normal distribution,the independent sample t-test was used between the two groups,the One-way Anova was used between the three groups,and the paired sample t-test was used for the intra-group comparison,and the difference between P<0.05 was statistically significant.Results:1.MCP-1(P<0.05),G-CSF(P<0.05),TNF-α(P<0.05),IL-5(P<0.05),IL-6(P<0.05),IL-7(P<0.05)and IL-10(P<0.05)expression levels were higher than those in the healthy group;IL-2 was lower than in the healthy group,and a statistically significant difference(P<0.05)indicated that all seven factors were related to the disease,MCP-1,G-CSF,TNF-α,IL-5,IL-6,IL-7 and IL-10 were positively associated with the disease,and IL-2 was negatively associated with the disease.2.Compared before and after the intervention of traditional Chinese medicine in the HSPN group,it was found that there were statistical differences in the levels of cytokine content in the serum before and after the treatment of traditional Chinese medicine in the HSPN group(P<0.05),MCP-1(P<<0.05),G-CSF(P<0.05),TNF-α(P<0.05),IL-5(P<0.05),IL-6(P<0.05),IL-7(P<0.05)and IL-10(P<0.05).Expression levels decreased after MCP-1,G-CSF,TNF-α,IL-5,IL-6,IL-7,and IL-10 treatment,and elevated expression levels after IL-2 treatment.After treatment,it was found that MCP-1(P>0.05),G-CSF(P>0.05),TNF-α(P>0.05),IL-5(P>0.05),IL-6(P>0.05),IL-7(P>0.05),and IL-10(P>0.05)。 After treatment,the expression levels of seven cytokines MCP-1,G-CSF,TNF-α,IL-2,IL-5,IL-6,IL-7 and IL-10 returned to normal levels,indicating that the intervention of chinese medicine Thistle Kidney Kang decoction had a regulatory effect on cytokines.3.One-way ANOVA was used to analyze the expression levels of MCP-1,G-CSF,TNF-α,IL-2,IL-5,IL-6,IL-7 and IL-10 in hematuria,proteinuria and hematuria plus proteinuria,MCP-1(P>0.05),IL-2(P>0.05),IL-7(P>0.05),IL-10(P>0.05),G-CSF(P<0.05),TNF-α(P<0.05)and IL-6(P<0.05).It is speculated that the expression levels of MCP-1,IL-2,IL-7,and IL-10 may not be significantly related to typing;the expression levels of G-CSF,TNF-α,and IL-6 in the hematuria plus proteinuria group are significantly higher than those of other groups,and it is speculated that G-CSF,TNF-α,and IL-6 may be related to the degree of renal damage.4.The results of the efficacy showed that the HSPN group was divided into hematuria(8 cases),proteinuria(6 cases)and hematuria plus proteinuria(2 cases),all of which observed 1 efficacy.Efficacy Results: 4 cases were cured,1 case was effective,1 case was effective,and0 cases were ineffective in the hematuria group.There were 5 cases of cured in the proteinuria group,3 cases of effectiveness,and 0 cases of effective and ineffective.One patient was cured,1 case was effective,and 0 cases were effective and ineffective in the hematuria plus proteinuria group.In this study,the healing rate was 62.5%,the apparent efficiency was31.25%,the effective rate was 6.25%,and the inefficiency rate was 0%.The efficacy of traditional Chinese medicine in the treatment of purpura nephritis is remarkable.5.Interleukin cytokines IL-2,IL-5,IL-6,IL-10 correlation analysis results show that the comparison results of IL-5 and IL-10 show that P>0.05,and the comparison results of IL-5and IL-2 show that P>0.05,IL-2 and IL-6 results showed P<0.05,IL-6 and IL-10 results showed P<0.05,IL-5 and IL-6 results showed P<0.05,IL-2 and IL-10 showed P<0.05.IL-2 is not correlated with IL-5,IL-5 and IL-10 in HSPN,and it is speculated that there may be no obvious correlation in HSPN disease;IL-2 is associated with IL-10,IL-5 and IL-6,IL-6 and IL-10,and may be involved in the occurrence of disease synergistically;IL-2 is significantly associated with IL-6,and it is speculated that it may be involved in the occurrence of HSPN.Conclusion:1.MCP-1,G-CSF,TNF-α,IL-2,IL-5,IL-6,and IL-10 are involved in the development of HSNP disease.2.MCP-1,IL-2,IL-7,IL-10 level expression may not be significantly associated with typing; G-CSF,TNF-α,and IL-6 may be related to the degree of renal damage.3.Thistle kidney kang decoction has obvious efficacy on purpura nephritis,and it is speculated that the efficacy targets of traditional Chinese medicine intervention are related to MCP-1,G-CSF,TNF-α,IL-2,IL-5,IL-6,and IL-10.
