Professor Zhang Mingxue's Study On The Syndrome And Treatment Rules And Network Pharmacology In The Treatment Of Stable Angina Pectoris Combined With Insomni

Purpose:Based on data mining and network pharmacology to summarize the medication rule of Professor Zhang Mingxue for treatment of stable angina pectoris combined with insomnia,and disuuss the mechanism of the basic prescription,provide new ideas and methods for TCM treatment of this disease.Material and method:Collect clinical data of patients with stable angina pectoris complicated with insomnia treated by Professor Zhang Mingxue from January 2019 to December 2020,extract relevant data and establish the database.Excel is used for statistical analysis of patients’ syndrome frequency,general information,prescription medication frequency,sexual taste meridian and drug efficacy.IBM SPSS Modeler 18.0 and IBM SPSS Statistics 26 software are used for association rules and cluster analysis.According to the above statistical analysis results,summarize my tutor’s syndrome treatment rules for stable angina pectoris combined with insomnia and get the basic prescription.TCMSP and Pharmmapper database are used for obtaining the information of effective components and targets of the basic prescription drugs,and the corresponding gene names of drug targets are obtained by Uniprot database,and the drug-component-target interaction network is constructed by Cytoscape 3.8.2.The targets of stable angina pectoris and insomnia are retrieved by Genecards and TTD disease target database,and Venny 2.1.0 is used for making venny diagram and obtaining the intersection targets of disease and drugs.Genomic ontology(GO)and Kyoto Encyclope-dia of Genes and Genomes(KEGG)pathways enrichment analysis of the intersection targets is performed by the Metascape database.To explore the mechanism of my tutor’s basic prescription for the treatment of this disease.Results:1.Distribution of syndrome elements: In the 215 patients,the main syndrome elemens are qi deficiency(98.14%),phlegm-dampness(97.21%),qi stagnation(73.49%).2.Medication pattern: A total of 139 kinds of traditional chinese medicine are involved in the215 prescriptions,and the 10 drugs with the highest frequency are Bupleurum root,Astragalus,Radix curcumae,American ginseng,Spina date seed,Allium macrostemon,Pinellia ternata,Snake gourd fruit,Rhizoma cyperi,Dragon bone.The main effects of drugs are benefiting vital energy,resolving phlegm,regulating qi,calming nerves.The medicinal properties are mainly flat,warm and slightly cold,the medicinal flavor are sweet,spicy and bitter.The drugs mainly belong to the lung meridian,heart meridian and liver meridian.3.Association rules: The second-order drug combination which has the highest support is American ginseng and Astragalus,while the third-order drug combinations which has the relatively high support is(1)Rhizoma cyperi,Bupleurum root,Radix curcumae.(2)Pinellia ternata,Snake gourd fruit,Allium macrostemon.4.Cluster analysis: A total of 4 drug combinations are obtained by cluster analysis,respectively:(1)American ginseng,Astragalus,Pinellia ternata,Snake gourd fruit,Allium macrostemon.(2)Rhizoma cyperi,Bupleurum root,Radix curcumae.(3)Spina date seed,Polygala tenuifolia willd.(4)Dragon bone,Poria cocos.5.Based on the data mining results and experience of the tutor,the basic prescription contains American ginseng,Astragalus,pinellia ternata,Snake gourd fruit,Allium macrostemon,Rhizoma cyperi,Bupleurum root,Radix curcumae.6.A total of 97 basic prescription effective ingredients are obtained from network pharmacology study,including β-sitosterol,quercetin,kaempferol,etc.There are 56 intersection targets of drugs and diseases,and the key targets are ALB,IL6,AKT1,TNF,IL1 B,VEGFA,CASP3,MMP9,TP53 and PPARG.GO enrichment analysis mainly involve the response to lipopolysaccharide,the negative regulation of response to extemal stimulus,response to radiation.KEGG pathway enrichment results show that genes are mainly enriched in AGE-RAGE signaling pathway in diabetic complications and IL-17 signaling pathway.Conclusion:1.Through data mining research,it is found that stable angina pectoris with insomnia is located in the heart,and is closely related to the liver.The pathogenesis always belongs to the origin of qi deficiency,the origin of phlegm dampness and the origin of qi stagnation.The basic treatment method is to improve qi and reduce phlegm,regulate qi and calm spirit.The basic prescription contains American ginseng,Astragalus,Pinellia ternata,Snake gourd fruit,Allium macrostemon,Rhizoma cyperi,Bupleurum root,Radix curcumae.2.Through network pharmacology study,it is found that the basic prescription has the characteristics of multiple components,multiple targets and comprehensive action.Which may affect ALB,IL6,AKT1,TNF,IL1 B and other targets through including β-sito sterol,quercetin,kaempferol and other components,and inhibit the AGE-RAGE signali-ng pathway in diabetic complications and IL-17 signaling pathway to achieve the trea-tment effect of the disease.
