Objective:To observe the effect of Guhan Yuyang recipe on wound healing of diabetic skin ulcer rats,to explore its mechanismof promoting wound healing,and to compare the curative effectof gel and spray.Methods:80 healthy clean male SD rats were randomly selected as normal control group(hereinafter referred to as control group),and the rest were diabetic group.The diabetic model was induc-ed by intraperitoneal injection of streptozotocin.60 diabetic ratswere selected to make skin ulcer model together with 15 normalrats by using surgical method.The control group was treated with Vaseline gauze.Diabetic rats were randomly divided into model group,Guhan Yuyang prescription spray treatment group(hereinafterreferred to as spray group),Guhan Yuyang prescription gel treatment group(hereinafter referred to as gel group)and Kangfuxin liquid treatment group(hereinafter referred to as Kangfuxin group),with 15 rats in each group.They were treated with Vaseline gauze,Guhan Yuyang prescription spray,Guhan Yuyang prescription gel and Kangfuxin liquid for 21 days(once a day).The woundhealing rate of rats in each group was recorded by naked eyeobservation and digital camera,and the wound healing rate was calculated at 3,7,14and21 days.Granulation tissue sections of ratsin each group were taken at 7,14 and 21 days after treatment.HE staining was used to observe the pathological changes of granulation tissue,and the protein expressions of insulin-like growth factor-1(insulinlikegrowthfactor-1,IGF-1)and vascular endothelial growth factor(vascularendothelialgrowthfactor,VEGF)were detectedby immunohistochemistry.Results:(1)At 3,7,14 and 21 days after administration,the wound healing rate in the control group,spray group,gel group and Kangfuxin group was significantly higher than that in the model group(P<0.01),and that in the gel group was significantly higher than that in the spray group(P<0.01).There was no significant difference in wound healing rate between gel group,Kangfuxin group and control group(P>0.05).(2)HE staining showed that the wound in the model group suppurated and had inflammatory exudate,and the wound shrank slowly.Compared with the model group,fresh granulation tissue proliferated obviously,capillaries grew rapidly,the number of fibroblasts increased significantly,and inflammatory cell infiltration decreased significantly in the control group and gel group,followed by the spray group and Kangfuxin group.(3)The results of immunohistochemistry showed that with the passage of time,the protein expression level of IGF-1 in wound granulation tissue of each group reached the peak on the 7th day and then decreased gradually.Compared with the model group,the protein expression level of IGF-1in the other four groupsincreased significantly at each time point(P<0.05 or P<0.01).Compared with the control group,the protein expression level of IGF-1 in the gel group was higher than that in the control group at each time point(P<0.05 or P<0.01).On the 7th and 21 st day,the protein expression level of IGF-1 in the spray group and Kangfuxin group was lower than that in the control group(P<0.05 or P<0.01),but there was no significant difference among the control group,spray group and Kangfuxin group on the 14 th day(P>0.05).Compared with gel group,the protein expression of IGF-1 in thespray group and Kangfuxin group was significantly different at each time pointt(P<0.01),and all were lower than those in gel group.The protein expression level of VEGF in wound granulationtissue of each group gradually increased to the peak on the 14 th day and then decreased gradually.Except for the model group,the protein expression of VEGF in the other four groups was at a stable and high level.Compared with the model group,the protein expression level of VEGF in the wound tissue of the other four groups increased significantly at each time pointt(P<0.05 or P<0.01).Compared with the control group,there was no significant difference in the protein expression level of VEGF among the spray group,gel group and Fangxin group(P>0.05).Compared with the gel group,the protein expression level of VEGF in the Kangfuxin group was lower on the 14 th day(P<0.05),and that of VEGF in the spray group on the 21 st day(P<0.05).Conclusion:Guhan Yuyang recipe can effectively promote thewound healing of diabetic skin ulcer rats,and its therapeutic mechanism may be related to the up-regulation of theexpression of IGF-1 and VEGF,and the curative effect of gel is better than that of spray. |
