Discussion On The Clinical Application Of Aconite In Malignant Tumors Based On Literature And Professor Cao Jianxiong's Experienc

Objective: Based on the literature and the clinical experience of my tutor Professor Cao Jianxiong,this paper discusses the TCM theory and clinical application of Monkshood in the treatment of malignant tumor and related complications,and provides some help for the clinical application of Monkshood in the treatment of malignant tumor and related complications.Methods:(1)Traditional philology methods were used to study and collate the literatures related to the application of Monkshood in malignant tumors in China Journal Full-text Database(CNKI)in recent 12 years,and summarize the efficacy,dosage,drug indications and pharmacodynamic responses of Monkshood in malignant tumors.(2)Summarize professor Cao Jianxiong’s academic views and clinical experience in using monkshood through clinical consultation and instruction by his tutor.Results:(1)Literature research:(1)Effi cacy and Indication s.Monkshood can warm yang to make water,and treat lower extremity edema caused by cancerous pleural ascites and lymphatic reflux disorder;it can warm yang and dissipate knots,the accumulation of broken diseases,and dissipate masses.the syndrome is yang deficiency phlegm coagulation or yang deficiency toxin knot;can warm yang and dispel cold,clear the collaterals and relieve pain,treat cancer pain of yang deficiency and cold coagulation;can warm spleen yang,invigorate kidney yang,resolve phlegm-dampness,treat malignant tumor yang Deficiency of phlegm and dampness condensed;it can subdue fire and return to the kidney,and is used for the syndrome of deficiency fire rising,deficiency yang floating and deficiency yang sinking in advanced malignant tumors;it can warm the yang qi of the whole body,strengthen the body and solidify the surface,return the yang to save the iniquity,and treat malignant tumors.Tumor yang deficiency sweating syndrome and death yang syndrome;can warm yang,stimulate qi and blood metaplasia,treat bone marrow suppression and decreased immunity after radiotherapy and chemotherapy;can warm kidney and spleen,promote yang and change yin,and treat intractable diarrhea of malignant tumors;Can warm yang and replenish qi,and treat cancer-related fatigue.In addition,Monkshood is flexibly compatible,and on the basis of each doing its own way and performing its own duties,it restricts each other,prevents biases and corrects disadvantages,so that yin and yang can help each other and use each other’s roots to expand its efficacy and cure.Monkshood is compatible with Huanglian,Scutellaria,gypsum and other heat-clearing medicines,can calm down cold and heat,and treat malignant tumors with mixed syndrome of cold and heat;it is compatible with blood-activating medicines such as Angelica,Chuanxiong,safflower,red peony,can warm the kidney and activate blood,and treat bone metastases;The external medicines such as ephedra and asarum are compatible,can warm the yang and dispel the evil,and treat malignant tumors with evil suffocating in the shaoyin;when combined with the loess in the heart of the heart and dried ginger,it can warm the yang and stop bleeding,and treat the cancerous bleeding of the spleen-stomach deficiency syndrome or the deficiency-cold syndrome;Compatible with centipede,leech,scorpion and other worm medicines and Sanleng,Curcuma and other blood-activating medicines,it can warm yang,remove blood stasis and dredge collaterals,and treat lower extremity venous thrombosis in patients with malignant tumors;when combined with astragalus,it can warm yang and promote muscle,and deficiency of yang is further aggravated,which affects the function of the viscera;the cold evil is deep;the syndrome of geyang and yang;the high-dose formula of the formula is not effective for severe cancer patients,or the dose needs to be reduced after the best curative effect is achieved.during maintenance therapy.Large doses are used for: dead yang syndrome;Sanyin cold-damp condensation,severe pain;Gradually increase the amount used in: pursuit of the best curative effect;large deficiency of kidney qi.(3)Medication indications.The key lies in "distinguishing yin and yang",and the key points of syndrome differentiation are the appearance,shape,thirst,constipation,tongue and pulse.Among them,yang deficiency,cold syndrome and pain syndrome are typical indications of monkshood;when yang syndrome is dead,it is necessary to urgently cast monkshood;evil excess has both positive deficiency,heat syndrome and yang deficiency and damp-heat syndrome(dampness is heavier than heat),and it can also be used appropriately;True cold and false heat syndrome should be carefully identified and not treated by mistake.In addition,most physicians believe that monkshood cannot be used for patients with excess heat syndrome,yin deficiency and internal heat syndrome,and yin and blood deficiency,and a small number of physicians believe that monkshood has no absolute contraindications.It is widely used in the evidence of cold and heat deficiency.(4)Pharmacodynamic response.When monkshood acts on the body,it will produce special effects,including dizziness,fever,tumor shedding,and rash on the chest and back.(2)Professor Cao Jianxiong’s academic viewpoint and clinical experience in using Radix Aconiti Lateralis Preparata:(1)The application of Radix Aconiti Lateralis Preparata focuses on syndrome differentiation.(2)Radix Aconiti Lateralis Preparata decoction,depending on the amount.(3)Be well versed in toxicity,and advance and retreat with rules.(4)Flexible compatibility,increasing efficiency and reducing toxicity.(5)The classical prescriptions are used flexibly,and there are many innovations.Among them,the commonly used pairs of Aconitum carmichaeli used by Professor Cao Jianxiong include: Aconitum carmichaeli and Pinellia ternate warming yang to eliminate phlegm,Aconitum carmichaeli and dried ginger warming yang to dispel cold,Aconitum carmichaeli and Bupleurum carmichaeli warming yang to relieve depression,Aconitum carmichaeli and Poria cocos warming yang to induce diuresis,Aconitum carmichaeli warming yang to dredge collaterals,Aconitum carmichaeli and Os Draconis,and magnet to ignite fire and return to the source.Commonly used classic prescriptions containing Fuzi include Zhenwu Decoction for treating lower limb edema caused by lymphatic reflux disorder after gynecological malignant tumor operation,Fuzi Jingmi Decoction for treating abdominal pain with dampness due to cold coagulation in ovarian cancer,Fuzi Lizhong Decoction for treating stomach cancer and intestinal cancer due to spleen-stomach deficiency and cold syndrome,Wenpi Decoction for treating intestinal cancer due to spleen-kidney yang deficiency and cold accumulation due to stagnation,Qianyang Fengmei Pill for treating late-stage imbalance of yin and yang,and hyperactivity of yuan yang.Conclusion:(1)Monkshood,as a representative medicine for warming yang,is widely used in the clinical application of malignant tumor.(2)Monkshood has clear indications for medication and has a special pharmacodynamic response.As long as the disease and pathogenesis are accurately grasped in clinical and rational use,it can bring better clinical efficacy to patients.(3)Different doses of monkshood have different effects.Clinically,it should be flexibly used according to the degree of yang deficiency and the needs of the disease.For physicians and scholars who use monkshood for the first time,it is recommended to follow the principle of "start with a small dose,gradually increase,take knowledge as the degree,and reduce the moderate disease immediately".For physicians with rich clinical experience,as long as the diagnosis is accurate and the condition requires,they can boldly invest.(4)Professor Cao Jianxiong has profound traditional Chinese medicine background,focuses on syndrome differentiation when using monkshood,and is good at breaking conventional rules,flexible compatibility,flexible use of classics,and good at advance and retreat.These unique views and experiences are worth learning and reference for the younger generation.
