The Clinical Curative Effect And Lipidomics Analysis Of "the Method Of Strengthening The Spleen And Kidney And Dredging Collaterals" On IgA Nephropathy

OBJECTIVE: To analyze the risk factors of poor efficacy in patients with IgA nephropathy after using the method of invigorating the kidney and dredging collaterals;and to compare the differences in serum lipid metabolites between the groups at baseline through lipidomics techniques,and to further explore the predictability of traditional Chinese medicine in the treatment of IgA nephropathy Lipid metabolites after disease progression.Methods: A single-center retrospective study was used to select primary IgA nephropathy diagnosed by renal puncture pathology.The patients were treated in Longhua Hospital of Shanghai University of Traditional Chinese Medicine and used Chinese medicine for kidney tonifying and collaterals to control the condition.Those who met the standard of intake and discharge,and the number of follow-ups ≥2 A total of 150 patients with an interval between two follow-ups of ≥3 months(90 days)were collected.The baseline data of the patients were collected,and the e GFR of the patients was estimated according to the CKD-EPI formula;the end-point event was a 50% decrease in e GFR compared with the baseline,and the kidney was switched on.Alternative treatment.Use regression analysis to analyze the risk factors of patients with renal function progression and end-point events;compare the differences in serum lipid metabolites between different curative effects and different renal functions through PCA,and further explore the lipid metabolites that can predict different progression of IgA nephropathy treated with traditional Chinese medicine.Results: Regression analysis and ROC curve analysis were used to analyze the baseline clinical indicators of patients with IgA nephropathy treated by the method of invigorating the kidney and dredging collaterals.The risk factors for the end-point event were found.The total hemorrhage protein and albumin were correlated,and theblood albumin and blood albumin were correlated with each other.The lower the total protein,the greater the risk of treatment failure or end-point events.PCA analysis and heat map show that there are differences in lipids between patients with different renal functions.However,further analysis is needed to clarify the structure of different lipid components and their pathways and their roles in diseases.Conclusion: Among patients treated with IgA nephropathy by the method of nourishing the kidney and dredging collaterals,the low levels of total protein and albumin before TCM treatment may indicate that IgA nephropathy is still progressing after treatment.In addition,there are differences in lipids between patients with different renal functions.
