ObjectiveTo explore the characteristics of knee muscle strength and gait spatio-temporal parameters in patients with knee osteoarthritis(KOA)and whether there is a relationship between them,and to explore the effect of internal heat acupuncture on patients with knee osteoarthritis.MethodsFrom July 2020 to October 2022,45 subjects were included from Peking University International Hospital,including 15 patients with knee osteoarthritis,15 middle-aged and elderly healthy people and 15 young healthy people.The isokinetic muscle strength test system and Gaitboter gait analysis system were used to test the knee flexion and extension muscle strength and gait space-time parameter characteristics of the three groups,and the correlation between the two patients with KOA was analyzed.In addition,9 KOA patients were selected from the subjects for 2 weeks of acupuncture fat pad thermal acupuncture treatment.The WOMAC scale and gait analysis were used to observe the effect of internal thermal acupuncture on KOA.ResultsKnee flexion and extension knee muscle strengthThe PT value and H / Q of isometric contraction of quadriceps femoris at 60 ° / s and180 ° / s,and the PT value of isometric contraction of hamstring at 60 ° / s in KOA group were significantly lower than those in young group.The PT value of 60 ° / s isokinetic contraction of quadriceps femoris and hamstring in KOA group was significantly lower than that in control group(P < 0.05).Gait space-time parametersThe walking speed of KOA group was significantly lower than that of control group and young group(P < 0.05),and the stride length was significantly lower than that of young group(P < 0.05).There was no significant difference between FPA and GRF groups(P >0.05).Correlation between knee flexion and extension muscle strength and gait spatiotemporal parametersThe walking speed of KOA group was positively correlated with the PT value of isometric contraction of quadriceps femoris,60 ° / s quadriceps femoris,60 ° / s and 180 ° / s hamstring isokinetic contraction(P < 0.05).The stride length of KOA group was positively correlated with the PT value of isometric contraction of quadriceps femoris,60 ° / s and 180 °/ s isokinetic contraction,and the PT value of hamstring 60 ° / s and 180 ° / s isokinetic contraction(P < 0.05).In KOA group,GRF was moderately positively correlated with PT value of 60 ° / s and 180 ° / s hamstring isokinetic contraction(P < 0.05).Therapeutic effect of internal heat needleThere was no significant difference in gait speed,stride length,foot angle and impact force between KOA patients before and after treatment(P > 0.05).The total score of WOMAC scale and knee joint function were significantly lower than those before treatment(P < 0.05).There was no significant difference in pain and stiffness subscales before and after treatment.Conclusion1.The muscle strength of quadriceps and hamstring muscles in KOA patients was significantly lower than that in healthy people and young people of the same age;the pace of KOA patients was significantly lower than that of healthy people and young people of the same age,and the stride was significantly lower than that of young people.The gait speed and stride length of KOA patients were significantly positively correlated with quadriceps muscle strength and hamstring muscle strength,and GRF was significantly positively correlated with hamstring muscle strength.2.The treatment of internal heat acupuncture can improve the knee joint function of KOA patients in a short period of time.
