Objective: To study the rules and patterns of exosomal miRNA-21 detection combined with PET/CT in the diagnosis of benign and malignant pulmonary ground glass nodules,to increase the non-invasive examination methods for the diagnosis of pulmonary nodules,and to provide scientific research direction for improving the early diagnosis rate of lung cancer.Methods: Forty patients with benign or precancerous lung lesions and 40 patients with malignant ground glass nodules were selected.According to the principle of consistency,PET/CT examinations were performed separately,and the imaging characteristics and SUVmax of nodules were counted.At the same time,5ml peripheral venous blood was collected from the patients.Transmission electron microscopy,nanoparticle size analysis and Western blot were used to identify the extracted serum exosomes,and real-time fluorescence quantitative polymerase chain reaction was used to detect the value of miRNA-21.The data obtained were analyzed using SPSS25.0.Results:1.The mean values of serum exosomal miRNA-21 in benign and precancerous lesions group and malignant group were 40.57±16.45pg/ml and128.63±34.79pg/ml,respectively,and the difference was statistically significant(P<0.05).The mean SUVmax of the benign and precancerous lesions group and the malignant group were 1.62±0.50 and 1.84±0.50,respectively.The SUVmax of the malignant group was higher than that of the benign and precancerous lesions group,but there was no statistically significant difference(P>0.05).2.Subgroup analysis of different imaging characteristics of malignant nodules showed that there was a statistically significant difference between the subgroup of mixed density nodules and pure ground glass nodules(P<0.05).Subgroup analysis of different nodule sizes showed that the mean value of serum exosomal miRNA-21 was statistically significant(P<0.05),and the SUVmax value was not statistically significant(P>0.05).There was no significant difference in the exosome detection value and SUVmax of the other subgroups(P>0.05).Conclusion: There is a statistically significant difference in serum exosomal miRNA-21 levels in the differential diagnosis of benign and malignant pulmonary ground glass nodules,which has certain clinical significance for the diagnosis.Because the uptake of 18F-FDG in GGNS is small and does not increase significantly with time,the value of SUVmax in the differential diagnosis of benign and malignant nodules needs to be further studied.However,in malignant nodules,SUVmax tends to increase with the increase of solid component of nodules,which may have certain clinical significance.
