Construction Of Clinical Practice Index System Of ICU Specialty Nurses Training In Yunnan Province

Objective(s):1.The purpose of this study was to investigate the status of intensive care unit(ICU)specialist nurse training in Yunnan province and to conduct a semi-structured interview with ICU specialist nurses to understand the training needs and deficiencies,and then through the delphi expert consultation method,to construct the index system of clinical practice ability training for ICU specialty nurses in Yunnan Province,and to provide reference for the standardized training of ICU specialty nurses in Yunnan Province.2.To formulate the clinical practice training manual for ICU specialty nurses and promote the standardized,systematic and scientific training of ICU specialty nurses in Yunnan Province.Methods:1 To understand the status quo of ICU specialty nurse training at home and abroad by consulting literatures;2 To understand the status quo of clinical practice training of ICU specialty Nurses in Yunnan Province by questionnaire survey;3 Through qualitative research(semi-structured interview)to understand in-depth the needs and deficiencies of ICU specialty Nurses’ clinical practice training in Yunnan province;4 Based on the results of Literature Review,questionnaire and semi-structured interview,the first draft of the clinical practice ability training system for ICU specialty nurses in Yunnan province was constructed.5 Through two rounds of delphi expert consultation,the index system was revised and improved,the weight of items was calculated,and the final draft of the practice training system for ICU nurses in Yunnan Province was constructed.Results:1 A total of 375 ICU nurses who had been trained in Yunnan province were investigated.A total of 372 effective questionnaires were collected,The effective recovery was 99.2%.Women accounted for 90.3 percent of the total,85.5% had worked in ICU for 2-10 years,93.3% were nurses and charge nurses,and 85.4% had bachelor degree or above.253 patients(68%)participated in the clinical practice for 1month,307(82.5%)needed ICU specialist nurses,and 306(82.3%)needed ICU specialist nurses,274(73.7%)were very clear about the objective of clinical practice training,255(68.5%)were very clear about the content of clinical practice training,and 242(65.1%)were very satisfied with the effect of clinical practice training.2 After an in-depth interview with 14 nurses who participated in the training of ICU Specialty Nurses in Yunnan Province,five themes were extracted according to the status quo,needs and deficiencies of ICU specialty nurses training:(1)clinical practice skills improved significantly;(2)the content of clinical practice training needs to be unified standard;(3)clinical practice time is insufficient;(4)the teaching faculty ability affects the learning effect;(5)the assessment standard needs to be more strict.3 Based on literature review,questionnaire survey and semi-structured interview,6primary indicators,26 secondary indicators and 42 primary indicators were formed after the study group discussion.The consultation was conducted by 19 experts from Grade III,Class A hospital and medical schools in five different provinces and cities.The basic situation of experts were as follows:clinical medical experts(21.1%),clinical nursing experts(47.4%),clinical education experts(5.3%),and clinical management experts(26.2%).Among them,10(52.63%)had bachelor’s degree,6(31.58%)had master’s degree and 3(15.79%)had doctor’s degree.There were 15 deputy senior titles(78.95%)and 4 senior titles(21.05%).The effective recovery rate of the first round questionnaire is 95%,and the expert authority coefficient is 0.905.The effective recovery rate of the second round questionnaire is 100%,and the expert authority coefficient is 0.914.The Kendal coordination coefficients of the first,second and third grade indexes were 0.189,0.306 and 0.354 respectively,P < 0.05.The first round of expert consultations resulted in the addition of 12 secondary entries,the revision of 3 secondary entries and the addition of 12 tertiary entries.No changes were made after the second round of expert consultations,which resulted in a convergence of views among all experts,resulting in six primary indicators and 36 secondary indicators,the Clinical Practice Ability Training Index system of 54three-level indexes for ICU specialty nurses.Conclusion(s):1 The construction of the clinical practice training index system of ICU Specialty Nurses in Yunnan Province is Operability,and the weight setting is scientific and reasonable.2 The“ICU Nurse Clinical Practice Training Manual” is worth promoting and using for reference.
