Comparative Study On Perinatal Outcome Of Singleton Pregnancy And Natural Singleton Pregnancy After In Vitro Fertilization And Embryo Transfer

Objective(s):Compared with natural singletons in the same period,the perinatal outcomes of patients with singleton pregnancy after in vitro fertilization and embryo transfer(invitrofertilization-embryotransfer,IVF-ET)were analyzed and compared,such as general condition,basic diseases before pregnancy,mode of delivery,pregnancy complications,neonatal outcome,etc.,to explore the effect of in vitro fertilization and embryo transfer(IVF-et)on perinatal outcome and the reasons for its differences from the point of view of obstetricians.The purpose of this study is to better manage the pregnancy and delivery of patients after IVF-ET,and to obtain better maternal and fetal outcome.Methods: The patients who were successfully conceived after in vitro fertilization and embryo transfer and delivered singletons in the Obstetrics-Reproductive Medicine Center of the affiliated Hospital of Yunnan University from December 2016 to December 2022 were divided into in vitro fertilization and embryo transfer group(IVF group).The age range was 20-40 years old.According to the same age principle of 1:1 in IVF group,300 pregnant women with singleton delivery in the same period of natural pregnancy were randomly selected as control group(natural group).The maternal information was obtained from the medical record management system of the affiliated Hospital of Yunnan University,and the pregnancy and delivery times,basic diseases,this pregnancy,delivery and newborn of the two groups were analyzed retrospectively,and the pregnancy complications,perinatal outcomes and birth defects of the IVF group and the natural group were compared by statistical methods.Results:(1)General condition: the average number of pregnancy,average parity and the proportion of past abortion(mainly induced abortion)in the IVF group were lower than those in the natural group(P < 0.05,0.05),and the pre-pregnancy BMI in the IVF group was significantly higher than that in the natural group,but there was no significant difference in BMI before delivery and weight gain during pregnancy between the two groups.(2)In terms of delivery mode,the proportion of cesarean section in the IVF group was higher than that in the natural group(P < 0.05,statistically significant).Among the people undergoing cesarean section,the proportion of "scar uterus" in the natural group was higher than that in the IVF group,while the proportion of fetal distress,failure and no indication of cesarean section in the IVF group was higher than that in the natural group.The proportion and average amount of postpartum hemorrhage in the IVF group were significantly higher than those in the natural group.(3)In terms of basic diseases before pregnancy,the proportion of thyroid diseases(mainly subclinical hypothyroidism)in the IVF group was significantly higher than that in the natural group(P < 0.05).There was no significant difference in other aspects such as chronic hypertension,diabetes mellitus and abnormal uterine development between the two groups(P > 0.05).(4)In terms of pregnancy complications,the incidence of threatened abortion in IVF group was higher than that in natural group,and the incidence of gestational diabetes mellitus and hypertensive disorder complicating pregnancy in IVF group was significantly higher than that in natural group(P < 0.05).There was no significant difference in other common pregnancy complications between the two groups(P > 0.05).In terms of abnormal placental formation,the proportion of placental accreta and abnormal placental morphology(racket placenta,sail placenta,etc.)in the IVF group was significantly higher than that in the natural group.(5)In terms of neonatal outcome,there was no significant difference in average neonatal weight,full-term low birth weight and macrosomia between the two groups,but there were 16 cases of birth defects in the IVF group,accounting for 5.3%,which was significantly higher than that in the natural group(2%),but there was no significant difference in the types of birth defects between the two groups.In terms of neonatal asphyxia,there was no significant difference between the two groups,and there was no statistical significance.Conclusion(s): After limiting the age factor and excluding the influence of multiple pregnancy,the risk of adverse perinatal outcome in the IVF group was higher than that in the natural group.Specifically reflected in the IVF group in threatened abortion,postpartum hemorrhage,non-indicative cesarean section and gestational diabetes,gestational hypertension,placental morphology abnormalities and the proportion of placental accreta,the proportion of neonatal birth defects;in terms of basic diseases,the proportion of thyroid diseases(mainly subclinical hypothyroidism)in the IVF group was higher than that in the natural group.
