Correlation Of Osteocalcin With Non-proliferative Retinopathy In Type 2 Diabetes

Objective(s):By comparing the differences and correlations of osteocalcin(OC),glucose,lipids,uric acid(UA),and blood pressure among patients with type 2diabetes mellitus(T2DM)and T2 DM non-proliferative retinopathy(NPDR)without chronic complications,we investigated the correlation between OC and the above indicators and NPDR,aiming to provide a theoretical basis for the factors influencing the occurrence and prevention of DR.Methods: One hundred and ten patients(n=110)with inpatient type 2 diabetes mellitus(T2DM)were selected,and based on clinical data and fundus examination results,T2 DM without chronic complications were divided into SDM group(n=53)and T2 DM with NPDR were divided into DR group(n=57).),body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose(FBG),2-hour postprandial blood glucose(2h PBG),glycated hemoglobin(Hb A1c),fasting C-peptide(FCP),total cholesterol(TC),triacylglycerides(TG),low-density lipoprotein(LDL),high-density lipoprotein(HDL),triacylglycerol glucose(Ty G),UA,serum urea nitrogen(BUN),serum creatinine(Scr),urinary microalbumin/urinary creatinine(ACR),estimated glomerular filtration rate(e GFR),and OC,and the differences between groups of the above indicators were compared;the correlation between OC and the above indicators was analyzed by one-way correlation;the factors influencing the occurrence of DR were analyzed by binary logistic regression.Results:(1)Among the T2 DM patients,the percentages of smoking,alcohol consumption,MS,poorly controlled Hb A1 c,overweight,dyslipidemia,HTN,HUA,and OC reduction accounted for 21.81%(24/110),9.09%(10/110),87.27%(96/110),81.81%(90/110),60.0%(66/110),and 82.72%(91/110),45.45%(50/110),15.45%(17/110),53.63%(59/110);compared with the SDM group,the DR group had older age,longer duration of DM,higher percentage of SBP,Hb A1 c,ACR,HTN,percentage of poorly controlled Hb A1 c,percentage of reduced OC,(P < 0.05),and lower FCP,e GFR,and OC(P < 0.05).(2)The level of OC was lower in the DR group than in the same-sex control group(P < 0.05),and the percentage of OC reduction was higher than in the same-sex group(P < 0.05).(3)The percentage of DR gradually increased and the OC level gradually decreased among the three groups of DM disease duration <5 years,5-10 years and ≥10 years,and the difference was statistically significant(P < 0.05).(4)OC was positively correlated with FCP(r=0.192,P<0.05)and HDL(r=0.231,P<0.05)in the SDM group,and with the duration of DM disease(r=-0.018,P<0.05),Hb A1c(r=-0.236,P<0.05),BMI(r=-0.194,P < 0.05),SBP(r =-0.240,P < 0.05),and ACR(r =-0.272,P < 0.05)were negatively correlated.patients in the DR group had a negative correlation between OC and SBP(r =-0.187,P < 0.05).(5)OC was a protective factor for the occurrence of DR in all T2 DM patients(OR=0.045,P<0.05),and the duration of DM disease≥10 years(OR=13.170,P<0.05),BMI≥24Kg/m2(OR=5.393,P<0.05),SBP≥140mm Hg(OR=9.720,P<0.05)were risk factors for the occurrence of DR.Conclusion(s):(1)Hospitalized T2 DM and DR patients are often combined with disorders of glucose,lipids,UA metabolism and hypertension,overweight,metabolic syndrome.(2)OC reduction is common in patients with T2 DM and DR,and the degree of OC reduction is more prominent in patients with DR.(3)The disease duration≥ 10 years,overweight,SBP ≥ 140 mm Hg may be risk factors for the occurrence of DR in patients with T2 DM,and normal OC may be a weak protective factor for the occurrence of DR.
