Objective:To explore the correlation between platelet parameters and inflammatory indicators of Retinopathy of prematurity(ROP)and the severity of ROP,as well as the predictive factors of ROP occurrence in early postbirth,in order to provide a basis for timely identification of ROP in clinical practice.Method:A total of 122 premature infants admitted to the Department of Neonatology,the Second Affiliated Hospital of Kunming Medical University from April 1,2022,to January 30,2023,were selected for ROP screening.According to the severity of ROP,they were divided into ROP group requiring treatment,ROP group without treatment,and control group.The differences in general clinical indicators,blood routine,inflammatory indicators,and complications among different groups were compared.Subsequently,binary Logistic regression analysis was used to analyze the data,identify the independent influencing factors of the occurrence of ROP,and establish a regression model.The model was evaluated by ROC curve and calibration curve.Results:(1)A total of 122 premature infants were enrolled,including 10 cases in the ROP group,27 cases in the ROP group without treatment,and 85 cases in the Non-ROP group.The average gestational age of ROP treatment group was 27.57(27.14,27.86)w,30.14(28.14,32.79)w for children without treatment,and 33.57(32,34.86)w for non-ROP children,and statistical differences were observed between groups(P<0.05).The average birth weight of children requiring treatment for ROP was1083.00±281.26 g,the average birth weight for children without ROP was1451.30±424.44 g,and the average birth weight for children without ROP was1765.65±383.00 g,and there were statistical differences between groups(P<0.05).There was a statistically significant difference between the 1-minute Apgar score of6(5,7)in children requiring treatment for ROP and 8(7,8)in children without ROP(P<0.05).The average 5-minute Apgar score for children requiring treatment for ROP was 7(7,7),the average score for children without ROP was 9(8,9),and the average score for children without ROP was 9(8,9),and there were statistical differences between groups(P<0.05).(2)The mean volume of red blood cells(MCV)in the ROP group,the ROP group without treatment,and the ROP group without ROP were 113.11±4.97,116.29±8.88,110.55±5.67 f L(P<0.001),and platelet count(PLT)was 199.40±27.28,207.11±75.47,256.01±65.29(P=0.001),respectively.The mean platelet volume(MPV)was 9.02±0.46,10.70±2.40,9.51±0.81(P<0.001),and the platelet volume(PCT)was 0.19±0.01,0.21±0.05,0.25±0.06(P=0.001),respectively.(3)The neutrophil-lymphocyte ratio(NLR)of children in the ROP group,the ROP group without treatment and the ROP no ROP group were 3.63±5.40,2.88±2.71,1.94±1.66,respectively,and the differences between the groups were statistically significant(P=0.041).(4)The proportion of ROP treatment group sepsis is higher than that in the N-ROP group,the proportion of anemia is higher than that in ROP and N-ROP without treatment,and the proportion of respiratory distress syndrome and sepsis in the ROP group without treatment is higher than that in the N-ROP group,and the difference is significant(P<0.05)。(5)Logistics regression showed that greater mean red blood cell volume(OR=1.107,95%CI:1.019-1.203,P=0.016)and large neutrophil-to-lymphocyte ratio(OR=1.29,95%CI:1.019-1.634,P=0.035)were independent risk factors for the occurrence of retinopathy of prematurity,and the greater gestational age at birth(OR=0.688,95%CI:0.548-0.865,P=).0.001),spontaneous delivery(OR=0.222,95%CI:0.064-0.775,P=0.018),and large platelet count within 24 hours of life(OR=0.985,95%CI:0.976-0.995,P=0.003,)were protective factors for retinopathy of prematurity.The area under the ROC curve of the model is 0.895,P<0.001,sensitivity is 0.865,and specificity is 0.795.The Hosmer-Lemeshow goodness of fit test was used to evaluate the calibration ability of the logistic regression model.The test showedχ~2value=9.442,P=0.306>0.05,which is greater than 0.05.Therefore,the logistic regression model has good calibration ability.Conclusion:(1)Gestational age and birth weight at birth were negatively correlated with the risk of ROP in preterm infants.The younger the gestational age and the lower the birth weight,the higher the incidence of ROP.(2)The younger the gestational age of the birth,the lower the birth weight,and the lower the Apgar score,the more serious the classification of ROP.(3)The average red blood cell volume,platelet count,mean platelet volume,and platelet volume may be associated with the occurrence and severity of ROP in preterm infants within 24 hours of life(4)Large average red blood cell volume and large neutrophil-to-lymphocyte ratio within 24 hours after birth were independent risk factors for ROP,and larger gestational age,spontaneous delivery and large platelet count within 24 hours after birth were protective factors for retinopathy of prematurity.(5)The model established by Logistic regression has a certain degree of differentiation and calibration.It can help identify children with ROP and has a certain clinical value.
