Objective:To create a reliable and valid assessment tool for the study of the mental health of Chinese resident trainees,it was necessary to construct it and test its validity and reliability.In order to provide a theoretical foundation for further improving resident trainees’mental health and addressing their mental health issues,a preliminary application of the Mental Health Rating Scale for Resident Trainees was carried out to investigate the factors influencing resident trainees’mental health and the association with their level of experience avoidance and mindfulness.Methods:Study 1:Development of the Resident Mental Health Assessment QuestionnaireThe structure and scale entry pool of the Mental Health Rating Scale for Resident Trainees were built based on the literature review and semi-structured interviews,using the conceptual framework of mental health and the mental health scales created by prior authors.In September 2021,the scale entries were qualitatively assessed using the Delphi expert consultation method,and the scale entries were changed based on the experts’assessments of the entries’significance and usefulness as well as their personal opinions.In November 2021,a residential training site was chosen in Kunming,Yunnan Province using a convenience whole-group sampling.A total of 386 valid questionnaires were gathered to screen the entries using exploratory factor analysis,extreme group comparisons,and correlations between question and total score.A residency training location in Kunming,Yunnan Province,received an approved version of the scale in September 2022,and a total of 650 valid questionnaires were gathered.Internal consistency coefficient,split-half reliability,validity scale validity,and validation component analysis were used to assess the performance of the scale.Study 2:Application of the Mental Health Rating Scale for Resident Trainees and Analysis of factors influencing mental healthThe trainees in Kunming,Yunnan Province,were surveyed using the self-administered"Mental Health Assessment Scale for Resident Trainees",the"Acceptance and Action Questionnaire–Second Edition"and the"Mindful Attention Awareness Scale"in order to determine their level of mental health,experience avoidance,and positive thought level.The 650 resident trainees who were the subject of the investigation were divided into independent samples,and the differences in the levels of their mental health were analyzed using the independent samples t-test and one-way ANOVA.Pearson product-difference correlation analysis and structural equation modeling(SEM)were then used to investigate the contribution of the level of positive thinking to the impact of experience avoidance on the mental health of resident trainees.Results:Study 1:Development of the Resident Mental Health Assessment QuestionnaireAccording to the quantitative analysis and screening of 386 questionnaires,the Resident Mental Health Assessment Questionnaire had 40 entries across 6 dimensions(stress perception and coping,somatic symptoms,clinical work errors,support network,training satisfaction,and emotional behavioral response),with a cumulative contribution to variance of 63.818%.The scale’s effectiveness was assessed using 650questionnaires,and the overall Cronbach’sαcoefficient was 0.954 with a split-half reliability of 0.887.The Cronbach’sαcoefficients for the individual dimensions ranged from 0.835 to 0.935 with a split-half reliability of 0.782 to 0.867,indicating that the reliability of the questionnaire was high.A validation factor analysis revealed that the model fit was good withχ~2/df=2.108(p<0.001),GFI=0.901,AGFI=0.876,NFI=0.930,RFI=0.917,CFI=0.962 and RMSEA=0.041.Study 2:Application of the Mental Health Rating Scale for Resident Trainees and Analysis of factors influencing mental healthThe results for the 650 residency trainees revealed scores of(102.34±24.43)for mental health,(22.09±8.42)for experiencing avoidance,and(65.70±13.17)for mindfulness.On the support network dimension,female students outperformed male students;on the clinical work dimension,graduate students outscored undergraduate students and demonstrated better clinical work processing skills;on the other five dimensions,first-year residency trainees underperformed except for the clinical work dimension.The level of mental health of the resident trainees is also influenced by whether or not they pass the license exam,if they are on rotation,their choice for the speciality,and the number of beds and shifts they oversee(all P values<0.05).Experiential avoidance was positively connected with the overall score of mental health level(r=0.684,P<0.01),and it was significantly positively correlated with the scores of all six aspects(all P values<0.01),according to Pearson correlation analysis.The degree of mindfulness was significantly adversely connected with the scores of all six characteristics(all P values<0.01)and with the overall score of mental health level(r=-0.653,P<0.01).According to SEM testing,mindfulness accounted for 32.33%of the overall effect of experiencing avoidance on mental health.Conclusions:1.The Resident Mental Health Assessment Questionnaire can be used to evaluate the mental health of trainee residents because it satisfies the psychometric standards with good reliability and validity.2.The mental health level was related to a number of variables in the application to a large sample of residency trainees,including gender,residency year,and residency type;experience avoidance was positively correlated with the total score of mental health level and the scores of all six dimensions,and the level of mindfulness was negatively correlated with the total score of mental health level and the scores of all six dimensions Experience avoidance not only affects their mental health directly,but also partly influences mental health through the mediation of mindfulness. |
