Background and purposeThe characteristics of induced abortion in China are large number,high rate of repeated abortion and large proportion of young women.The number of induced abortions is about 9 million every year.Medical abortion is one of the most recognized abortion methods for women.However,the success rate of medical abortion within 49 days of pregnancy is 90%.About 10% of women in early pregnancy need to face the problem of incomplete medical abortion or failure of medical abortion.Most women seek medical treatment because they are afraid of the pain of complete curettage of uterine cavity.Medication for incomplete medical abortion can not only successfully end this pregnancy,but also avoid the pain of patients undergoing complete curettage of uterine cavity.Although some studies have shown that the combination of mifepristone,misoprostol,estrogen,progesterone,and traditional Chinese medicine has a positive effect on the treatment of incomplete medical abortion,the clinical effect of the combination of drospirone ethinylestradiol(Ⅱ),mifepristone,misoprostol,and new biochemical granules is still unclear.Therefore,the purpose of this study is to explore the clinical effect of the combination of drospirone ethinylestradiol(Ⅱ),mifepristone,misoprostol and Xinshenghua granules in the treatment of incomplete medical abortion.MethodsPatients diagnosed as incomplete medical abortion in the family planning clinic of the Second Affiliated Hospital of Harbin Medical University from September 2020 to March 2021 were included.After introducing the drug and surgical treatment plan and relevant precautions to the patients in detail,the patients informed consent and volunteered to be included in the combined drug group and the curettage group.It is planned that 100 patients will be included in each group.Excluding those who did not strictly implement the drug regimen and those who lost follow-up,90 cases were actually enrolled in the combined drug group and 94 cases were actually enrolled in the curettage group.The combined drug group was treated with the combination of drospirone ethinylestradiol(Ⅱ),mifepristone,misoprostol and Xinshenghua granules,and the curettage group was treated with complete curettage of uterine cavity.The effect was judged one week after the treatment,and the cure rate and pain degree were counted.After the first resumption of menstruation,the continuous bleeding time of vagina,the recovery time of menstruation,and whether there is pelvic inflammatory disease are counted.ResultsThere was no significant difference in the cure rate between the combined drug group and the control group(94.44% vs 97.87%;P>0.05).The patients in the combined drug group had lower pain scores(4.0[4.0,5.0] vs 7.0[6.0,8.0];P<0.001),shorter menstrual recovery time(33.0[30.0,34.0] vs 37.0[34.8,39.0];P<0.05),but prolonged vaginal bleeding time(6.0[5.0,7.0] vs 4.0[4.0,5.0];P<0.05),There was no significant difference in the incidence rate of pelvic inflammatory disease between the two groups(1.11% vs 3.19%;P>0.05).ConclusionsIncomplete medical abortion was treated with the combination of drospirone ethinylestradiol(Ⅱ),mifepristone,misoprostol,and Xinshenghua granules.As far as the cure rate was concerned,the combined drug treatment was as effective as the complete curettage of uterine cavity,and the combined drug treatment could avoid the surgical pain and complications that women feared.Combined drug treatment can also shorten the recovery time of menstruation.Therefore,the drug treatment plan for incomplete medical abortion is a reliable and conservative alternative method for patients who do not want to perform complete curettage of uterine cavity.
