Objective(s):To explore the effect of TCM evidence-based diet and conventional dietary guidance intervention on improving the nutritional status of patients diagnosed with lung cancer undergoing chemotherapy for the first time.To explore the effect of TCM evidence-based diet combined with nutritional therapy on the nutritional status and appetite of lung cancer chemotherapy patients,and to develop a scientific and feasible evidence-based dietary guidance program to provide new ideas to improve the nutritional status and appetite of lung cancer chemotherapy patients and to enhance their tolerance during chemotherapy.Methods:According to the inclusion and exclusion criteria,88 patients diagnosed with lung cancer and treated for the first time with platinum-based chemotherapy regimens admitted to the Department of Integrative Medicine of a tertiary care oncology hospital in Yunnan Province from March to October 2022 were selected as study subjects and grouped according to the time of first admission using an experimental study-like method,including 46 patients in the control group and 42 patients in the intervention group.Patients in both groups received conventional therapeutic care and nutritional treatment,based on which the control group received conventional dietary guidance and the intervention group received TCM evidence-based dietary guidance.The patients’ subjective global nutritional assessment(PG-SGA),appetite symptom questionnaire(CASQ)and chemotherapy adverse reaction score were evaluated before and after the intervention,and the nutritional status,appetite symptoms and chemotherapy adverse reactions of the two groups were observed from the first chemotherapy to the end of 4 cycles of chemotherapy using blood routine and blood biochemical indexes.Results:A total of 75 patients eventually completed the study and had their data statistically analyzed(38 in the control group and 37 in the intervention group).The results were as follows:1.Comparison of general information,pre-intervention anthropometric indicators,routine blood and blood biochemical indicators,PG-SGA assessment,and CASQ scores between the two groups was not statistically significant(p> 0.05);2.Comparison of anthropometric indicators between the two groups: there was no statistically significant difference in weight and BMI after the intervention(p>0.05);the upper arm circumference of the intervention group improved better than that of the control group,and the difference was statistically significant(p=0.002);3.Comparison of routine blood and blood biochemical indexes between the two groups: after the intervention,erythrocytes,leukocytes,neutrophils,hemoglobin,platelets,and serum albumin were decreased in both groups,and serum albumin was increased,and the difference was statistically significant(p<0.05).After intervention,prealbumin increased in the intervention group,and the difference was statistically significant(p=0.048),while all other indexes were compared,and the difference was not statistically significant(p>0.05);4.Comparison of PG-SGA between the two groups: the difference in PG-SGA scores after the intervention was not statistically significant(p=0.436),and the reduction in PG-SGA scores in the intervention group was better than that in the control group,with a statistically significant difference(p=0.014);5.Comparison of CASQ between the two groups: there was no statistically significant difference in CASQ scores after the intervention(p=0.328).In the control group,the CASQ score decreased after the intervention,and the difference was statistically significant(p=0.018).There was no statistically significant difference between the pre-intervention scores and the post-intervention scores(p=0.567);6.Comparison of adverse reactions to chemotherapy between the two groups of patients: there was no statistically significant difference in adverse reactions to chemotherapy between the two groups of patients at different time points after the intervention(p>0.05).There was no improvement in the adverse reactions to chemotherapy in the control group after the intervention compared with that before the intervention,and the difference was not statistically significant(p=0.063).The intervention group showed improvement in adverse reactions to chemotherapy after intervention compared to pre-intervention,and the difference was statistically significant(p=0.004).Conclusion(s):1.Lung cancer chemotherapy patients have significantly reduced immune function after 4 cycles of chemotherapy treatment.Conventional dietary guidance combined with nutritional therapy and TCM evidence-based diet combined with nutritional therapy can improve the nutritional status of chemotherapy patients;2.The combination of TCM evidence-based dietary guidance and nutritional therapy improved the appetite of patients but did not differ significantly from conventional dietary guidance combined with nutritional therapy in improving the adverse effects of chemotherapy in lung cancer chemotherapy patients;3.The combination of TCM evidence-based diet and nutritional therapy is scientifically effective and worthy of clinical promotion,which can be used as a reference to carry out personalized dietary guidance for patients undergoing chemotherapy and improve the efficacy of comprehensive treatment.
