Objective:The objective is to evaluate the efficacy and safety of feces-washing bacteria transplantation combined with traditional Chinese medicine(TCM)intervention on behavioral symptoms, gastrointestinal symptoms and sleep disorders in children with autism. The clinical efficacy of washing flora transplantation in the treatment of autism was analyzed as a whole without grouping, the clinical efficacy of washing flora transplantation in the treatment of autism with or without constipation, and the clinical efficacy of washing flora transplantation combined with or without traditional Chinese medicine in the treatment of children with autism were analyzed.Method:Patients who were admitted to the Department of Gastroenterology, the First Affiliated Hospital of Guangdong Medical University from June1,2019 to October 31,2021,and retained the medical record data of two or more groups of autistic patients after treatment with washed flora transplantation. The baseline data of ABC,CARS,and SDSC,BSFS and ABC,CARS,SDSC and BSFS after WMT were analyzed by self-made and pre-made control to obtain the pre-made and post-made differences. The children were then divided into the constipation group and the normal group, and the difference in scale scores between the two groups was compared. The scale scores after baseline and the first, second, third, fourth and fifth WMT were compared and statistically analyzed to explore the difference in scale indexes between the constipation group and the normal group after baseline and WMT.According to whether the patients in the treatment group received TCM treatment during the WMT period, they were divided into TCM intervention group and control group, and the difference in scores between the two groups was statistically analyzed. To compare the routine blood test, liver and kidney function, and intestinal barrier function indicators before and after WMT in children, and evaluate the safety of WMT.Statistical analysis was performed by SPSS26.If P was less than 0.05,statistical difference was considered. Results:A total of 71 patient sample data were collected.Compared with the baseline scores, the scale scores of ABC,CARS and SDSC in children were decreased. After the first WMT,there was no statistical difference in ABC scale(P>0.05),but there were significant differences after the 2 nd,3 rd and 4 th WMT(P < 0.01).After the 1 st,2 nd,3 rd and 4 th WMT,there were significant statistical differences in CARS scale for children with autism(P=0.022 P=0.002 and P=0.001 P=0.008),suggesting that the autism symptoms of children in the group were better than those at baseline. The SDSC scale had a statistical difference after the 4 th WMT(P=0.047),suggesting that the sleep disorder of children with autism included in the group improved after the 4th WMT.There was no significant statistical difference in SDSC after the 1 st,2 nd,3 rd and 5 th WMT(P > 0.05).The BSFS after the 1 st,2 nd,3 rd and 4 th WMT had significant statistical differences(P=0.000),and the mean BSFS after multiple WMT increasingly approached to the healthy stool morphology level. There was no significant difference in the ABC scale after WMT for the first time in the constipation group(P=0.092),but there were significant differences after WMT for the second, third, fourth and fifth times(P=0.028 P=0.024 and P = 0.006 P = 0.024).In the normal group, there was no significant difference after the first and second WMTs of autism patients(P=0.268 P=0.107),and there was a significant difference after the third, fourth and fifth WMTs(P=0.003 P=0.018 and P = 0.035).The CARS scale for autism patients in the constipation group showed no statistical difference after the 1st and 5 th WMT(P=0.072 P=0.066),but showed statistical difference after the 2 nd,3 rd and 4 th WMT(P = 0.034 P = 0.034 P = 0.049).The CARS scale of patients in the normal group had no statistical difference after the first WMT(P=0.072),but had statistical differences after the 2 nd,3 rd,4 th and 5 th WMT(P = 0.043 P = 0.007 P= 0.019).The SDSC scale of patients in the constipation group had no statistical difference after the 1st,2nd and 3rd WMT(P=0.828 P=0.286 and P=0.187),and had statistical difference after the 4th and 5th WMT(P= 0.042 P = 0.008).The SDSC scale of patients in the normal group had no statistical difference after the 1 st and 2 nd WMT(P=0.26 P=0.1),but had statistical difference after the 3 rd,4 th and 5 th WMT(P=0.03P=0.018 and P=0.035).There were significant differences in CARS and BSFS scales between the TCM intervention group and the control group(P=0.036),while no significant differences were found in ABC and SDSC scales. There was no statistical difference in biochemical indicators,Diamine oxidase,D-lactic acid and bacterial toxin before and after WMT(P>0.05).Conclusion:The ASC,CARS and SDSC scale scores of children with autism were decreased, and the gastrointestinal symptoms and autistic symptoms were improved, proving that fecal-washing bacteria transplantation had a significant therapeutic effect on children with autism. After treatment, the typical autistic symptoms of children with autism were alleviated, and the gastrointestinal accompanying symptoms such as constipation and diarrhea were also improved.A sufficient course of WMT was significant for the treatment of sleep disorders in children. For children without gastrointestinal symptoms, the efficacy of fecal washout bacteria transplantation in the treatment of autism is more significant, suggesting that gastrointestinal disorders(constipation)is one of the interferences that affect the efficacy of WMT.WMT did not damage the liver and kidney function, routine blood test and intestinal barrier function of children with autism, confirming that WMT is relatively safe and reliable.
