ObjectTo observe the effect of Trauma Yellow Water on wound healing and TCM syndrome scores in patients with diabetic foot,to observe the content of methylglyoxal in serum of patients with diabetic Foot inflamed and dispel rotten Stage,to assess its the clinical efficacy and safety and to explore the intervention mechanism of Trauma Yellow Water in the treatment of diabetic foot.MethodsBased on the inclusion and exclusion criteria,72 patients with diabetic Foot inflamed and dispel rotten Stage admitted to the Department of Endocrinology of Foshan Hospital of Traditional Chinese Medicine between February 2022 and January 2023 were selected and allocated to control and experimental groups on the basis of the principle of randomisation.There were 36 cases each group.As part of the basic internal medicine treatment received by both groups,the traumatized skin was routinely disinfected and treated by debridement,and the experimental group’s patients were treated with Trauma Yellow Water combined with moist Aner’s iodine III dressing on the trauma surface and an external dressing with sterile gauze.Wounds from the control group were moist packed with Aner’s iodine type III and wrapped with sterile gauze.The dressing was changed once daily for 14 days in both groups,and 1 course of therapy was observed.Baseline data for patients in both group were collected at enrollment,and data on trauma area,depth of trauma,secretion culture status,MG serum level,safety index,blood glucose and clinical scoreswere observed and recorded pre-and post-treatment in both groups at the time of enrolment and at the completion of treatment.Lastly,the data observed above were statistically analyzed with the statistical software SPSS 27.0.To assess the clinical efficacy and safety of Trauma Yellow Water in the treatment of diabetic foot at inflamed and dispel rotten Stage.Results1.Baseline data:There was no statistical difference in baseline data such as sex,age,duration of diabetic foot,body mass index(BMI)and blood pressure in the study(P >0.05)at pre-treatment,which were found to be comparable.2.The area and depth of trauma:Before treatment,there was no statistically significant difference in the area and depth of trauma in the study(P>0.05),which was comparable;after treatment,there was a statistically significant difference in the area and depth of trauma between the2 groups(P<0.05).3.The detection rate of traumatic pathogens:There was no statistically significant difference(P>0.05)in the pre-treatment detection rate of traumatic pathogens between the two groups After treatment,the detection rate of traumatic pathogens was 19.4% in the treatment group and 43.8% in the control group,and the difference between the two groups was statistically significant following comparison(P<0.05).4.Serum MG:There was no statistically significant difference(P>0.05)in the pre-treatment serum MG levels between the two groups,and serum MG concentrations were decreased in both groups following treatment(P<0.05),and serum MG in the experimental arm was less than that in the controlgroup(P<0.05).5.Blood glucose outcome:Before and after treatment,the two groups had no statistically significant differences in fasting blood glucose(FBG)and 2-hour postprandial blood glucose(PBG)(P>0.05).6.Safety index:There was no statistical difference in the concentrations of alanine aminotransferase(ALT),aspartate aminotransferase(AST),urea(Urea)and blood creatinine(Cr)between the two groups before and after the treatment period(P>0.05).7.The clinical scores:The differences in the clinical scores of the two groups at pre-treatment were not statistically significant(P>0.05)and were comparable to each other.The difference between the 2 groups at post-treatment was statistically significant(P<0.05),and the experimental group had lower points for TCM syndrome compared to the control group(P<0.01).ConclusionTrauma Yellow Water’s effectiveness in the treatment of diabetic foot is remarkable.in the inflamed and dispel rotten Stage.It may reduce the area of the wound,decrease the depth of the ulcer,reduce the exudate from the wound,and promote the growth of granulation,resulting in an acceleration of wound healing.In addition,it can improve clinical symptoms such as pain,numbness and swelling in patients’ lower extremities.The presence of yellow water in wounds can reduce serum methylglyoxal(MG)levels in patients,suggesting that the mechanism of action of the yellow water injury may be linked to the reduction in serum MG. |
