| Background and purposePrimary duodenal adenocarcinoma(PDA)is a very rare malignant tumor in gastrointestinal tract.In recent years,the possibility of occurrence of PDA has obviously enhancd.Serum carbohydrate antigen 199(CA19-9)and serum carcinoembryonic antigen(CEA)elevated in some patients with primary duodenal adenocarcinoma,but their influence on the prognosis and recurrence of PDA are still controversial.In this research,the associated data of 80 PDA patients with PDA who taken radical surgical treatment in our hospital from January 2014 to December 2018 were collected,and the associated with risk factors of PDA patients after surgery and the impact of CEA and CA19-9 levels on make a forecast of patients before operation were counted and summarized,which provided theoretical basis for evaluating patients’ early diagnosis,disease monitoring and prognosis judgment.MethodsFrom January 2014 to December 2018,the clinical data of 80 patients with PDA treated by radical surgery in our hospital were collected,including gender,age,tumor invasion depth,lymph node metastasis,distant metastasis,tumor differentiation degree,pathological type,gross type,tumor location,tumor diameter,peritoneal metastasis,peripheral organ metastasis,intravascular tumor thrombus and nerve invasion.SPSS26.0 program was put to used for statistical analysis,and P<0.05 was looked upon as statistically significant.ResultsThe increase of serum CEA concentration is related to patients’ age and lymph node metastasis(P<0.05),the increase of serum CA19-9 concentration is related to patients’ gender(P<0.05),and the correlation analysis between serum CA19-9concentration and lymph node metastasis is P = 0.057.Multivariate analysis showed that the increase of serum CEA concentration before operation was related to lymph node metastasis(P < 0.05)The increase of serum CA19-9 concentration before operation was related to the gender of patients and lymph node metastasis of tumor,with P values less than 0.05.The 1-year,2-year,and 3-year survival rates of CEA elevated group were 83.3%,58.3%,and 16.7%.The 1-year,2-year and 3-year survival rates of normal CEA concentration group were 85.2%,57.3% and 27.9%,with no significant difference.The 1-year,2-year and 3-year survival rates of CA19-9 group were 85.1%,46.8% and 14.9%.The 1-year,2-year and 3-year survival rates of normal CA19-9 concentration group were 84.8%,72.7% and 42.4%,P<0.05,the difference was statistically significant.The 1-year,2-year and 3-year survival rates of patients with elevated serum CEA and CA19-9 concentrations before operation were 80.0%,50.0%and 10.0%.The 1-year,2-year and 3-year survival rates of patients with normal serum CEA and CA19-9 concentrations before operation were 83.9%,71.0% and 41.9%,with no significant difference.ConclusionsThe increase of serum CEA and CA19-9 concentration before operation is related to lymph node metastasis,and the increase of serum CA19-9 concentration before operation is an unfavorable factor for the prognosis of patients. |
