Objective: The results of gastric cancer screening in Handan City from October 2019 to April 2020 were analyzed,and the risk factors associated with gastric cancer were researched to determine the risk factors of such kind of disease.Based on the risk factors of gastric cancer,knowledge education and health guidance are carried out for high-risk groups,so as to enhance the public ’s risk awareness and reduce the incidence of gastric cancer in Handan from the root.Methods: Cluster sampling method was adopted in Handan area.Firstly,residents aged 40-69 years old were selected as research object to initially screen out high-risk groups.Then,gastroscopy screening and diagnosis were carried out in high-risk groups,and discussed the screening results.For the sample size of screening is small,non-screening patients who underwent gastroscopy at the Western Gastroscopy Center of Handan Central Hospital from 2019.10 to 2020.4 were collected.According to gastroscopy and pathological diagnosis,chronic non-atrophic gastritis and gastric adenocarcinoma were divided into control group(N=657)and experimental group(N=38).The corresponding clinical information of them were collected based on follow-up,such as age,gender,smoking history,drinking history,drinking water source,eating habits(whether they often eat pickled food,fried food,hot food,vegetables,fruits),whether they have a family history of gastric cancer and BMI.All data were collated and spss23 statistical software was used.Firstly,chi-square test was applied to determine the significant factors.Then,multivariate regression analysis were conducted on these significant variables and to calculate the indexes of odds ratio(OR)and 95% CI.The P< 0.05 were regarded as significant criteria,and finally the risk factors related to gastric cancer were obtained.Results: 1.A total of 4071 people participated in the questionnaire for gastric cancer screening,including 1624 high-risk people,with a high-risk rate of about 39.9%.The actual number of people who completed gastroscopy was 1287,and the screening rate was 79.2%.The number of pathological biopsy was 545,and the pathological screening rate was42.3%.There were 365 patients with inflammatory changes,41 with atrophy,94 with intestinal metaplasia,33 with low-grade intraepithelial neoplasia,9 with gastric cancer.The detection rate of precancerous lesions was 2.8 %,the screening rate of early cancer was 25 %,and the detection rate was 0.7 %.Inflammatory lesions were obviously significant in age,while gastric cancer in age and gender.2.Univariate analysis showed that age,gender,smoking,often eat pickled food,often eat vegetables and fruits,have a family history were related to the occurrence of gastric cancer,which were suspicious factors,and there exist obvious difference(P<0.05).Multivariate regression result reflected that age(OR=12.078,95%CI:1.107-8.889),gender(OR= 3.190,95 %CI: 1.029-9.895),drinking(OR =2.801,95 % CI: 1.021-7.686),eating pickled food(OR = 4.893,95 % CI:1.950-12.280),and having a family history of gastric cancer(OR=5.259,95 % CI: 2.386-11.589)were associated with gastric cancer.The OR value was greater than 1.May be an independent risk factor.Conclusion: 1.In the screening of gastric cancer in Handan City,age and inflammatory changes were statistically significant,and age and gender were statistically significant with gastric cancer.2.Among the related risk factors of gastric cancer,advanced age,male,regular drinking,regular eating pickled food,and family history of gastric cancer are independent risk factors for gastric cancer in Handan City.3.The above risk factors have important guiding significance for suggesting the occurrence and development of gastric cancer in Handan City,and health education can be focused on the above factors. |
