| Part I: Construction of HIF-1α targeted knockout mice model and study of its influence on periodontitis and atherosclerosisObjective: Using Cre/Lexp system builds hypoxia inducible factor-1αConditionally knock out the chronic periodontitis and atherosclerosis models of mice,and verify whether the animal model is successfully constructed.The expression of vascular endothelial growth factor A in mice was detected to study HIF-1α and the possible effects of VEGFA on periodontitis and atherosclerosis.Methods: HIF-1α Gene Loxp labeled mice and Isl1-Cre ERT2 transgenic mice were hybridized to obtain Isl1-Cre positive and HIF-1α homozygous Loxp labeled mice were induced with Tamoxifen to obtain HIF-1α Conditional knockout mice.Thirty-two mice were randomly divided into four groups: group A normal control,group B local application of lipopolysaccharide to construct periodontitis model,group C high-fat diet to construct atherosclerosis model,group D local application of lipopolysaccharide and high-fat diet to construct periodontitis combined with atherosclerosis model.After death,samples were observed the pathological changes of periodontal tissue,the levels of,Total Cholesterol,Triglyceride and VEGFA were detected by ELISA,and the pathological changes of periodontal tissue and arterial blood vessels were observed by histopathology.Results: 1.PCR agar gel electrophoresis results showed that the Isl1-Cre positive and HIF-1α Homozygous Loxp labeled mice.2.Periodontal clinical observatio and periodontal tissue pathological observation showed that the periodontitis of mice in Group B and Group D was inflammatory and alveolar bone resorption was lost.3.The results of ELISA showed that the concentrations of TC and TG in serum of group C and D mice were higher than group A(P<0.05).4.Histopathological observation of coronary artery showed that the blood vessel wall of group C and group D mice was thicker than that of group A,foam cells were formed,and muscle fibers were in disorder.5.The expression of serum VEGFA protein in group B,C and D was significantly higher than group A(P<0.05).Group B,C and D mice achieve HIF-1αunder induction,the expression of VEGFA in serum after conditional knockout was significantly lower than the control group(P<0.05).Conclusion: 1.The HIF-1α conditional knockout periodontitis combined with atherosclerosis model mice were successfully constructed.2.Periodontitis and atherosclerosis may interact and promote each other.3.HIF-1α plays an important role in the occurrence and progression of the two diseases by controlling the expression of VEGFA.PartⅡ:Clinical cases presentation of Periodontal endoscope and laser diode assisted periodontal treatmentObjective: By showing periodontal assisted by endoscope and semiconductor laser on periodontal basic treatment of periodontitis patients and to evaluate the curative effect of periodontal basic treatment and periodontal endoscope and auxiliary effect of semiconductor laser.Methods: Included in accord with the new classification of Ⅲ period B/C level of patients with periodontitis,contraindications,for patients with periodontal endoscope and semiconductor laser assisted basis of periodontal therapy for periodontal therapy.At the initial visit and six months follow-up,photo collection in patients with clinical mouths,using periodontal probing diagnosis and inventory records of Florida periodontal clinical indices and analyzed.Periodontal clinical indices including plaque index,gingival sulcus bleeding index,probing clinical hemorrhage,explore clinical depth and degree of loose teeth and root bifurcation lesions,etc.Through the analysis of clinical periodontal indexes before and after treatment,in order to evaluate the efficacy of periodontal basic treatment and periodontal endoscope and auxiliary effect of semiconductor laser.Results: This cases presentation included patients with periodontitis Ⅲ grade B/C of 5 people,all of the patients undergoing periodontal endoscope and semiconductor laser aided by periodontal basic treatment,periodontal diagnosis depth decreased obviously,clinical bleeding significantly improve,swollen gums hyperplasia varying degree improved,tooth mobility degree improved significantly,degree of molar root bifurcation lesions improved to different extent.5 patients with periodontal probing diagnosis to begin with a depth of 3.4 mm or site of full mouth periodontal probing diagnosis site between 52.5 to 71.4%,the proportion of the average of 64.08 ±8.73%.Positive explore clinical bleeding site proportion reached full mouth periodontal probing diagnosis site in 63.1 to 96.4%,the proportion of86.72 ±13.41% on average.Six crescent weeks postoperatively in patients with out attending a depth of 3.4 mm or more loci of between 3.7 to 38.9%,the average of17.41±13.14%.Periodontal probing positive clinical bleeding sites between 6.2 to20.4%,16.01±5.82% on average.According to the results,in the six months after treatment,patients with cross periodontal probing depth of 3.4 mm or more sites with the initial consultation has declined significantly by around 40%.All depth average agent clinical periodontal patients have different degrees of lower,in addition to the individual sites,most of the dentine diagnosis depth have been reduced to normal levels.All patients with positive explore clinical bleeding site of comparing with the initial consultation also has significantly reduced,the sharpest decline among them 4patients,up to 84.5%.Conclusion: This cases presentation included all of the patients during the six months of follow-up periodontal status has improved significantly,periodontal basic treatment have good curative effect.For patients with periodontitis Ⅲ B/C grade in periodontal assisted by endoscope and semiconductor laser periodontal basic treatment in the short term treatment effect is remarkable,tooth mobility and root bifurcation lesions,red,also different degree improved gingival hyperplasia. |
