| Objective(s):Coronary atherosclerotic heart disease(CHD)is one of the major diseases endangering human health in the world. The incidence rate and mortality of coronary heart disease in China are also gradually increasing, which brings a heavy burden to China.Chronic coronary syndrome refers to the different development stages of coronary heart disease except for the clinical manifestations dominated by acute coronary artery thrombosis. It is a dynamic process of atherosclerotic plaque accumulation and changes in coronary artery circulation function. It can delay or reverse the disease progress by adjusting lifestyle, drug treatment and blood flow reconstruction. The general practice model is patient-centered,providing targeted and comprehensive treatment programs for patients, and providing health education, lifestyle guidance, psychological guidance, drug guidance, etc. This study is to explore the effect of managing patients with chronic coronary syndrome with general practice on negative emotions of anxiety and depression, cardiac function and major adverse cardiovascular events, and to preliminarily analyze and evaluate its effect.Method(s):(1)Based on the inclusion criteria and exclusion criteria,175 CCS patients were selected from the Affiliated Hospital of Guilin Medical College inpatient from July 2020 to July 2021,the patients were divided into observation group(N = 83)and control group(N=92)according to the results of telephone or outpatient follow-up.The control group was treated with routine diagnosis and treatment after discharge, while the observation group was treated with general medicine. The cardiac function, anxiety, depression and major adverse cardiovascular events of the two groups were observed.(2)Observation indicatorsCardiac function indexes:left ventricular ejection fraction(LVEF),left ventricular end diastolic dimension(LVDd),left atrial end systolic dimension(LASd),ventricular septal thickness(IVST).Evaluation indicators of the scale:Self-rating anxiety scale(SAS)and Self-rating depression scale(SDS)scores.Occurrence of major adverse cardiovascular events(MACE):As an evaluation index of the prognosis of patients, the incidence rate is calculated based on the number of major adverse cardiovascular events during the period, including hospitalization of angina pectoris, acute myocardial infarction, heart failure, and death of coronary heart disease.(3)The effect of general practice management model on CCS patients was analyzed by statistical methodResult(s):(1)Comparison of baseline data:83 patients in the observation group and 93 patients in the control group were compared with the baseline of the demographic, sociological and clinical data of the patients in the two groups. The gender difference was statistically significant(P<0.05),and the other differences were not statistically significant(P>0.05).(2)Comparison of results of transthoracic echocardiography:before management, there was no statistically significant difference in LVEF,LVDd,LASd and IVST between the two groups(P>0.05);After management: LVDd,LASd and IVST had no statistically significant difference(P>0.05),while LVEF had statistically significant difference(P<0.05).(3)Comparison of anxiety and depression scores:after the management of general practice, the scores of anxiety and depression in the control group were higher than those in the observation group, and the difference was statistically significant(P<0.05).(4)Comparison of major adverse cardiovascular events(MACE):After general practice management, the incidence of adverse cardiovascular events in the observation group was lower than that in the control group, the differences were not statistically significant(P>0.05).Conclusion(s):Management of CCS patients in general practice mode can improve the cardiac function, anxiety and depression of patients, and may reduce the incidence of major adverse cardiovascular events, thus improve the quality of life of patients, for the management of CCS patients to play a guiding role. |
