Objective:The expression levels of n CD64 index,IL-6 and hs-CRP in bacterial infection of premature infants were analyzed,and the diagnostic value of n CD64 index combined with IL-6 and hs-CRP in bacterial infection of premature infants was discussed,providing evidence for improving clinical diagnosis rate,guiding treatment and monitoring efficacy.Methods:The preterm infants were divided into infected group and non-infected group,and the infected group was further divided into perinatal high-risk factor group,local infection group and severe infection group.Blood samples of all included subjects were collected at admission,at the time of confirmed infection of suspected infected persons,and on the 5th day after anti-infection treatment to detect n CD64 index,IL-6 and hs-CRP,and receiver operating characteristic(ROC)curve was drawn.Receiver operating characteristic(ROC)curve was drawn.Excel and SPSS were used to compare the diagnostic value of n CD64 index,IL-6,hs-CRP and combined test in bacterial infection of premature infants.Result:A total of 125 premature infants were included,including 152 blood samples,including 51 in the non-infected group,30 in the perinatal high-risk factor group,38 in the local infection group and 33 in the severe infection group.1.The mean index of n CD64 in the infected group was 4.62(2.59,8.45),and that in the non-infected group was 1.36(0.63,2.37).The value of n CD64 in the infected group was significantly higher than that in the non-infected group,and the difference was statistically significant(p<0.01).2.The n CD64 index of perinatal high-risk factor group,local infection group and severe infection group before and after treatment were 4.63(2.89,8.21)vs 1.41(0.94,1.83),3.06(2.20,10.15)vs1.12(0.54,1.92),5.84(3.07,7.78)vs 1.44(0.82,2.10),with statistical significance(all p <0.01).3.The area under the curve of the n CD64 index is 0.876,the standard error is 0.027,and the 95% confidence interval is 0.823-0.929.When the n CD64 index =2.775,the sensitivity is 73.3%,and the specificity is 86.3%.4.The sensitivity of the alliance with n CD64 index /hs-CRP,IL-6/hs-CRP,n CD64 index /IL-6,n CD64 index /IL-6/hs-CRP for the diagnosis of bacterial infection in premature infants were 93.1%,92.1%,95.0% and 98.0%,respectively.The specificity was 47.1%,45.1%,58.8% and 35.3%,respectively.Positive predictive values were 77.7%,76.9%,82.1% and 75.0%,respectively.Negative predictive values were 77.4%,74.2%,85.7% and 90.0%,respectively.Accuracy: 77.6%,76.3%,82.9%,77.0%,respectively.Conclusion:1.n CD64 index is closely related to bacterial infection in premature infants.2.n CD64 index has diagnostic value in the diagnosis and treatment of bacterial infection in premature infants;Pairwise union assay of n CD64 index or IL-6 had the higher diagnostic value in the early stage of bacterial infection in preterm infants;The alliance of n CD64 index or IL-6 or hs-CRP had the highest sensitivity in the diagnostic value of bacterial infections in preterm infants.
