Study On Platelet Count Function In Patients With Iron Deficiency Anemia

Objective:To study the changes of platelet quantity and function in patients with iron deficiency anemia(Iron deficiency anemia IDA),and to explore the treatment strategy of iron deficiency anemia with thrombocytosis.Methods:112 IDA patients in our hospital from January 2021 to December 2022 were selected as the experimental group and 40 healthy volunteers as the control group.The differences of blood cell count(RBC,Hb,PLT),platelet aggregation function(MAR)and iron metabolism indexes(SI,SF)between the two groups were compared and analyzed.According to the platelet count,iron deficiency anemia was divided into two groups:PLT≤300×109/L was the group with no increase in platelets,and PLT>300×109/L was the group with increase in platelets.The changes of RBC,Hb,PLT,MAR,SI and SF were compared between the two groups.The IDA patients were given iron supplementation,and the changes of the above indexes were observed after treatment in the thrombocytosis group.Results:1.General features:There are 112 patients with Iron deficiency anemia,including 34 males and 78 females,with a ratio of 3:7,with an average age of 39.06±14.02 years.There were 40 cases in the control group,including 10 males and 30 females,the ratio of male to female was 1:3,and the average age was 36.50±8.57 years.2.Blood cell count:in IDA group:RBC4.13±0.51×1012/L,HB81.48±12.58g/L,PLT309.16±102.49×109/L.Control group:RBC4.48±0.19×1012/L,HB 129.55±5.77 g/L,PLT222.40±42.75×109/L.RBC and Hb in IDA group are lower than those in control group,and the difference was significant(P<0.001).PLT is higher than that of the control group,and the difference was significant(P<0.001).3.Platelet aggregation function:MAR 30.88±9.98%in IDA group and MAR 57.51±10.80%in control group.MAR in IDA group is lower than that in control group,and the difference was significant(P<0.001).4.Iron metabolism index:IDA group:SI3.50±1.29umol/L/L,SF6.35±3.41ug/L.SI18.71±5.73umol/L and SF94.84±44.13ug/Lin the control group.SI and SF in IDA group are significantly lower than those in control group(P<0.001).5.According to the platelet count,iron deficiency anemia was divided into two groups:platelet-elevated group and platelet-non-elevated group.5.1 Blood cell count:RBC4.15±0.40×1012/L,HB 79.81±11.33 g/L and PLT412.76±115.29×109/L in the elevated group.Non-elevation group:RBC4.12±0.56×1012/L,HB82.31±13.14g/L,PLT258.05±35.75×109/L.There was no significant difference in RBC and Hb between the two groups(P>0.05).There was significant difference in PLT(P<0.001).5.2 Platelet aggregation function:MAR of the elevated group was28.28±8.52%,while MAR of the non-elevated group was 32.16±10.45%.There is significant difference in MAR between the two groups(P<0.05).5.3 Iron metabolism index:SI3.20±1.14umol/Land SF5.16±3.53ug/Lin the elevated group.Non-elevation group:SI4.18±1.42umol/L,SF6.44±3.36ug/L.There is significant difference in SI and SF between the two groups(P<0.05).6.Changes of indexes in PLT increased group after 2-3 weeks of treatment:6.1 Blood cell count:RBC4.46±0.22×1012/L,HB 110.05±14.33g/L,PLT286.41±43.28×109/L.After treatment,RBC and Hb increased and PLT decreased,which is significantly different from that before treatment(P<0.001).6.2 Platelet aggregation function:MAR47.79±17.38%.After treatment,MAR increased significantly(P<0.001).6.3 iron metabolism indexes:SI9.52±2.98umol/L,SF52.03±9.80ug/L.After treatment,SI and SF increased significantly(P<0.001).Conclusion:1.The platelet count of patients with iron deficiency anemia is normal or increased,and the increase of platelet has nothing to do with the degree of anemia.2.When iron deficiency anemia is accompanied by an increase in platelet count,platelet aggregation function tends to decrease.3.Iron deficiency anemia with increased platelet count needs no special treatment.After iron supplementation,platelet count can return to normal.
