Objectives:1.To investigate and analyze the current situation of long prescription management for chronic diseases,including the awareness of relevant personnel,patient satisfaction survey,and the problems of long prescription management,etc.2.To study the countermeasures and operation of the standardized management of long prescription for chronic diseases at home and abroad,and learn from the experience and reference3.To formulate countermeasures for standardized management of long prescriptions for chronic diseases through case studies,including standardizing prescription standards,strengthening physician training and supervision,etc.,and establishing a tracking and evaluation mechanism for long prescriptions to continuously improve and optimize the management system.Methods:1.This study used questionnaire and interview method.2.The chronic patient population in several medical institutions in Wuhan was selected as the study population,and the types of chronic diseases of the surveyed population appeared in the questionnaire,and the questionnaire survey did not involve the non-critical patient population,and the final total number of questionnaires that met the criteria of nadir and the information was completed was 302.The interview method used semi-structured interviews,and the number of patients interviewed was 20 in total,and the number of hospital and community staff prescribing long term prescriptions was 5 in total.3.Self-administered questionnaires were used to collect data,with four sections.The general information section collected socio-demographic information of chronic disease patients,including eight questions on the participants’ gender,age,education level,monthly personal income,type of disease suffered,longest medical history,and average number of medical visits per year.The awareness section mainly investigated 16 questions from the two parts of medical insurance policy,and long prescription management of the examinees.The satisfaction section examined 15 questions from the respondents on five variables: patient understanding,medication effectiveness,long prescription health insurance reimbursement,service quality,and physician professionalism.The patient interviews mainly included long prescription awareness,medication management process,influencing factors in long prescription treatment,and long prescription health insurance reimbursement.The interview contents of hospital and community long prescription prescribers mainly included the cognition of long prescription policy,the supporting measures of long prescription,and the standardized management of long prescription,etc.4.The data of this study were analyzed by using SPSS 26.0 statistical software.Descriptive statistics were used to analyze basic patient conditions and patient satisfaction;univariate analysis was used to analyze the differences in attitudes toward long prescription cognition among medical personnel of different genders,ages,education levels,and occupational types,and multiple regression analysis was further used to analyze the factors influencing the cognition of chronic disease long prescription management under the health insurance policy.Result:1.The results of the univariate analysis showed that there was no statistically significant difference between gender on the perception of chronic disease long prescription management in health insurance policy(F=23.425,P>0.05);age(F=23.969,P<0.01),education level(F=7.000,P<0.01),and type of occupation on chronic disease long prescription management in health insurance policy(F=50.022,P<0.01).50.022,P<0.01),and comparing the different occupational types one by one shows that pharmaceutical and paramedical personnel have a higher degree of awareness compared to other practice types,and the difference in scores is statistically significant(P<0.05).2.The results of multiple linear regression analysis showed that the greatest influence on the perception of chronic disease long prescription management of health insurance policy was the type of occupation,followed by the medical institution,and then the education level and age.3.The results of satisfaction analysis showed that: chronic disease patients’ satisfaction with health insurance reimbursement for long prescriptions was in the middle to upper level;satisfaction with prescribing process was lower and satisfaction with payment for prescribing was higher among satisfaction with long prescription prescribing;satisfaction with long prescription physicians was lower;satisfaction with the degree of long prescription description was higher.4.The results of the interviews showed that the number of patients interviewed totaled20 and the number of hospital and community staff prescribing long prescriptions totaled 5.Patients had deficiencies in their knowledge of long prescription health insurance,lack of understanding of the long prescription drug management process and health insurance reimbursement,and dissatisfaction with the management of long prescription regulations.The hospital and community prescribing staff have a misunderstanding of the long prescription policy,are dissatisfied with the supporting measures for long prescriptions,and believe that the management of long prescription regulations needs to be improved.Conclusion:1.Type of occupation,choice of medical institution,education level,and age were important factors influencing the perception of health insurance policy and long prescription management for chronic diseases.2.Deficiencies in patients’ perceptions of health insurance policies and long prescription management,irregularities in the long prescription drug prescribing process,deficiencies in chronic disease health management,and the need to improve physicians’ satisfaction with long prescription management.3.Targeted recommendations are proposed,mainly including establishing a standardized service process for long prescription for chronic diseases,improving the integration of health management and chronic disease patients,improving the management mode of long prescription drugs for chronic diseases by medical insurance,and introducing an information support system for the management of chronic diseases by medical insurance.
