Objective:To investigate the effect of blood uric acid levels on cognitive function in long-term hospitalized schizophrenic patients;Exploring factors affecting cognitive function in long-term hospitalized schizophrenic patients;Method:Eighty-four long-term hospitalized patients with schizophrenia(study group)and 64 healthy individuals(control group)who voluntarily participated in this study and were able to complete biochemical tests and neurocognitive scale assessments consistently were selected as inpatients at the Third People’s Hospital of Foshan City from September 2020 to September 2022,according to the 2013 version of the Chinese Expert Consensus on Hyperuricemia(HUA)Diagnostic Criteria,according to fasting serum uric acid levels were divided into hyperuricemia group(fasting blood uric acid levels:uric acid(UA)> 420 mmol/L in men and uric acid(UA)> 360mmol/L in women)and normal uric acid group.The scales included:self-administered general questionnaire,determination of fasting uric acid level,blood lipids,fasting glucose,thyroid function and cortisol using a fully automated biochemical analyzer;psychiatric symptoms were assessed using the Chinese Positive and Negative Syndrome Scale(PANSS),Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA).The Wisconsin Card Sorting Test was used to assess the cognitive function of both groups.Differences in cognition between the study groups were assessed using the Schizophrenia Cognitive Functioning Battery(MCCB).Result: 1.The number of completed classification and the number of correct responses in the normal uric acid group and the hyperuricemia group were significantly lower than those in the normal control group,and the differences were statistically significant(F=30.823,F=30.634 均 P<0.001).The number of total responses and the number of incorrect responses were significantly higher in the normal uric acid group and the hyperuricemia group than in the normal control group,and the differences were statistically significant(F=11.975,F=29.938,both P<0.001).The hyperuricemia group had a statistically significant difference in the number of correct responses compared to normal uric acid values(P<0.001).2 The hyperuricemia group had a statistically significant difference in social cognition as well as total MCCB score compared to the normal uric acid group(F=11.43,F=5.865 均P<0.005).3 The higher the serum uric acid value,the higher the MCCB-social cognition score(t=2.512 P=0.014)the higher the fasting glucose value,the lower the MCCB-social cognitive score(t=-2.218 P=0.03).The higher the serum uric acid value,the higher the total MCCB-score(t=3.143 P=0.002);the higher the total PANSS-score,the lower the total MCCB-score(t=-2.411 P=0.018);the lower the total MCCB-score in the case of married relative to unmarried.The effect of HDL on MCCB social cognitive scores was not significant(B=0.8116,P=0.902),but the interaction between uric acid and HDL was significant(B=0.1994,P=0.002),with a moderating effect of HDL.There was no significant effect of HDL on MCCB social cognitive scores(B=0.251,P=0.970),and the interaction between uric acid and HDL was significant(B=0.1598,P<0.001),,indicating a moderating effect of HDL.The positive effect of uric acid on MCCB social cognitive scores was increased in patients with high HDL(double standard deviation above the mean)(B=0.0745,P<0.001)Conclusion : Long-term hospitalized schizophrenia patients have cognitive impairment and uric acid levels were found to be strongly associated with cognitive function in long-term hospitalized schizophrenia.The higher the PANSS total score,the poorer the cognitive function in long-stay schizophrenia.Total MCCB scores were also lower in the married condition relative to the unmarried.There may be a negative association between fasting glucose and social functioning.And HDL was found to act as a moderating effect that may modulate the effect of uric acid on cognitive function in long-term hospitalized schizophrenia,with HDL at high levels and an increased positive effect of uric acid on MCCB social cognition and total scores.
