Study On The Distribution Of Tcm Syndrome Elements And The Correlation Of Constitution In Short Children

Objectives: By formulating the syndrome factor information questionnaire and constitution questionnaire for children with short stature,the distribution of TCM syndrome factors and the correlation between constitution and syndrome factors in children with short stature were studied,which enriched the theoretical research of traditional Chinese medicine on children with short stature,and provided reference for the definition,etiology,pathogenesis,syndrome type and treatment of short stature in traditional Chinese medicine.Methods:1.Through the preliminary clinical investigation of 20 children with short stature who visited the pediatric outpatient and inpatient departments of Sichuan Provincial Hospital of Traditional Chinese Medicine,combined with the clinical experience of the tutor,the questionnaire items were adjusted,and the children ’s syndrome information questionnaire and constitution questionnaire were finally formulated.2.2.Children who underwent growth hormone stimulation test from the pediatric inpatient department of Sichuan Provincial Hospital of Traditional Chinese Medicine,and met the diagnostic criteria of short stature western medicine,and did not meet the exclusion and exclusion criteria were selected as subjects.A total of 109 cases were included.3.The collected clinical information and physical scores and other data were entered into Excel office software.After sorting out the data,according to the criteria of syndrome factor determination,the syndrome factor calculation table was used to obtain the syndrome factor information and determine the physical type.4.SPSS 25.0 software was used to analyze the data of disease location,disease nature syndrome elements and physical data,summarize the general data of short stature children,disease location,disease nature syndrome elements and physical type data,and analyze the above data.Results:1.A total of 16 effective TCM syndrome elements,7 disease location syndrome elements and 9 disease nature syndrome elements were extracted.The syndrome elements of disease location from high to low were spleen(36.84 %),kidney(21.05 %),lung(18.42 %),stomach(10.53 %),liver(6.58 %),heart(4.61 %)and large intestine(1.97 %).The syndrome elements of disease nature from high to low were qi deficiency(38.04 %),dampness(17.39 %),blood deficiency(8.70 %),essence deficiency(8.70 %),yin deficiency(7.61 %),fire heat(6.52 %),dyspepsia(5.43 %),yang deficiency(5.43 %),dryness(2.17 %).There were 63 times of deficiency disease nature,accounting for 68.48 %,and 29 times of solid disease nature,accounting for 31.52 %.Cluster analysis,clustered into 6 categories :spleen as a class,lung as a class;kidney,qi deficiency as a class;the liver is a class;stomach and dampness are a class,and other syndromes are a class;2.There are 9 types of constitution types,one type of gentleness(21.13 %),one type of biased constitution(78.87 %).Among the biased constitutions,qi deficiency(32.39 %),special constitution(17.61 %),phlegm dampness(10.56 %),damp heat(4.93 %),yang deficiency(4.23 %),qi stagnation(4.23 %),yin deficiency(3.52 %),and blood stasis(1.41 %)are in turn.3.Relationship between constitution and syndrome elements : Chi-square or Fisher exact probability was used to explore the relationship between disease location,disease nature and constitution.Children with different constitutions have different disease locations(the highest proportion of gentleness is spleen and stomach;qi deficiency is spleen;yin deficiency is kidney;qi stagnation constitution is liver),disease nature(Qi deficiency is the most in gentleness constitution,blood deficiency,essence deficiency and dyspepsia account for the same proportion;qi deficiency is Qi deficiency;phlegm dampness is dampness;yang deficiency is Yang deficiency).Conclusion :1.Children with short stature have the characteristics of multiple disease locations and multiple diseases.The disease locations are spleen,lung,kidney,stomach,liver,heart and large intestine.The nature of the disease is qi deficiency,dampness,blood deficiency,essence deficiency,yin deficiency,fire heat,dyspepsia,yang deficiency and dryness.The nature of the disease is mixed with deficiency and excess,mainly deficiency.The etiology,pathogenesis and treatment of children with short stature can be explored from the spleen,kidney,lung and stomach,and the disease can be explored from qi deficiency,dampness,blood deficiency and essence deficiency.2.Different disease locations and natures have certain differences in constitution.Different disease location of gentleness,qi deficiency,yin deficiency,qi stagnation of the four types of constitution,the difference was statistically significant(P < 0.05).Different diseases have statistically significant differences in the presence or absence of the four constitutional types of gentleness,qi deficiency,yang deficiency and phlegm dampness(P < 0.05).There was no significant difference between the other constitutions.3.Clinical treatment should pay attention to different zang-fu organs and diseases according to different constitutions,pay attention to individualized treatment,and spleen and qi deficiency are the focus of attention.
