| ObjectiveVaricocele is a condition of clinical dysfunction caused by pathological venous reflux,which can cause painful discomfort in the ipsilateral groin,scrotum and its contents,as well as progressive loss of testicular function,and is a common cause of male infertility.There are several surgical treatments available in Western medicine,but interventional embolization is currently highly respected in clinical practice as a minimally invasive approach.Knowledge of the anatomical variants at the spermatic vein drainage is the basis for the success of interventional embolization techniques.Many studies have now analysed the venous anatomical findings of the left spermatic vein,but less research has been done on the right spermatic vein.With the development and application of Multi-slice Computed Tompgraphy,due to its high spatial resolution and the advantages of image post-processing,the anatomical structure of the right spermatic vein can be studied by Multi-slice Computed Tompgraphyo improve the knowledge of the anatomical variation of the right spermatic vein and to provide clinical convenience.The anatomical structure of the right spermatic vein in patients admitted to the hospital was analysed using Multi-slice CT to investigate the distribution characteristics,typing and the value of the right spermatic vein for clinical application.Materials and methodsA cross-sectional study was conducted by retrospectively counting the imaging data of 1,000 men who visited the Affiliated Hospital of Chinese Medicine,Chengdu University of Traditional Chinese Medicine for Multi-slice Computed Tompgraphy examinations from April 2022 to October 2022.Based on the findings of Yoel Siegel et al,the right spermatic veins were classified into 6 categories,with the following data: 1)location,type and number of veins draining from the right spermatic vein;2)orientation angle of the right spermatic vein in the axial plane;3)diameter of the right spermatic vein anterior to the point of drainage;4)distance 1 and distance 2 of the right spermatic vein draining into the main trunk of the vein;5)right angle between the spermatic vein and the inferior vena cava.Results1.Duplicated spermatic veins were visible on Multi-slice Computed Tompgraphy(2.2%),with direct drainage into the inferior vena cava being the most frequent type(87.8%),and 90% of the drainage angle is below 25.7 °.2.Right spermatic veins were mostly in the third and fourth quadrants when in the axial plane,with 86.4% located between 240° and 360°.3.The diameter of the entrance to the right spermatic vein ranged from 2.7 to 3.8 mm When the right spermatic vein drains the inferior vena cava,it ranges from 19.6 to 36 mm,with 83% of the right spermatic veins located within 40 mm below the right renal vein orifice;the average distance from the main trunk of the vein when draining into the right renal vein is 6.3 mm,mainly between 4.4 and 6.9 mm.There was no significant correlation between the complexity of the right spermatic vein typing,size of the diameter,size of the pinch angle and length of the distance and the patient’s age(P > 0.05)。ConclusionsThe anatomy of the right spermatic vein can be assessed by Multi-slice Computed Tompgraphy.The anatomy of the right spermatic vein in adults is independent of age and there are more anatomical variations at the right spermatic vein drainage.By understanding these anatomical conditions,interventional cannulation of the spermatic vein can be facilitated,improving the success rate and reducing the operative time. |
