Objective: To explore the clinical efficacy,trauma and stress of Da Vinci robotic surgery system in the treatment of patients with differentiated thyroid cancer by comparing the surgical effect,postoperative complications and stress index of Da Vinci robotic surgery with traditional open surgery,and to lay the foundation for the clinical promotion of Da Vinci robotic surgery system.Methods:A retrospective study was conducted to analyze 199 patients with differentiated thyroid cancer who were admitted to Hospital from January 2018 to December 2021.Among them,185 patients were under 60 years old and were non-randomly divided into Da Vinci surgery group(RT group)and traditional open surgery group(COT group)according to the patients’ will.There were 75 patients in RT group and 110 patients in COT group.The clinical data of the patients were collected,including age,gender,BMI,tumor location,multifocality,etc.The operation time,intraoperative blood loss,postoperative drainage volume,drainage days,pathological type,capsule invasion,postoperative complications,postoperative stress index,postoperative pain score,postoperative cosmetic satisfaction score and other indicators were collected and analyzed.Results:1.The 185 young and middle-aged patients included in the study were analyzed.Compared with patients in the COT group,the operation duration and hospitalization cost of patients in the RT group increased significantly,while the amount of surgical bleeding decreased significantly,and there was a statistical difference(P <0.05),but there was no significant difference in tumor pathological results and the number of lymph nodes dissected(P> 0.05).2.Compared with COT group,the probability of postoperative parathyroid injury,transient hypocalcemia and complications in RT group is significantly lower than that in COT group,with statistical differences(P< 0.05).There were no significant differences in postoperative complications such as permanent hypocalcemia,numbness of hands and feet,temporary hoarseness,vocal cord paralysis and incision bleeding infection between the two groups(P>0.05).COT group was compared with RT group,the pain scores of day 1,day 2and day 3 after surgery were significantly reduced between the two groups,with statistical difference(P<0.05).There was no significant difference in stress indexes(IL-6,WBC,CRP)on day 1 and day 3 after surgery between the two groups(P>0.05).Postoperative paresthesia and aesthetic score of surgical incision in RT group were significantly reduced between the two groups,with statistical differences(P<0.05).Conclusions:1.Compared with traditional open surgery,Da Vinci robotic thyroid surgery is significantly superior to traditional open surgery in terms of surgical effect and postoperative complications.2.Da Vinci robotic surgery system is safe and effective in the treatment of differentiated thyroid cancer surgery.The surgical traumatic stress of Da Vinci robotic surgery system is similar to that of traditional open surgery,but the pain of Da Vinci robotic surgery system is lower than that of traditional open surgery,the cosmetic effect is more obvious,and the patient satisfaction is higher.
