Objective: To explore the correlation between garlic intake and gastric cancer under different Helicobacter pylori infection,I analyze the main factors affecting gastric cancer in Wuwei area,and construct a risk assessment model of gastric cancer,so as to provide theoretical basis for the prevention of gastric cancer in this area.Methods:(1)Based on the implementation plan of the National Health Commission’s project "screening and early diagnosis and early treatment of Upper Digestive tract Cancer",this project included 10035 people from 21 administrative villages in Liangzhou District of Wuwei City.The data collected by the project include physical examination data,health factor questionnaire,C14 breath test results,gastroscopy and pathological diagnosis results.Using case-control study,to compare whether there are statistical differences in sex,age,smoking,drinking,Helicobacter pylori infection,disease history,family history,garlic,whole grain,legume food intake frequency and other influencing factors between the gastric cancer group and the control group.Then the relationship between garlic intake and gastric cancer was analyzed according to the uninfected state of Helicobacter pylori,the infected state of Helicobacter pylori and the eradicated state of Helicobacter pylori.(2)Based on the Harvard cancer risk index model,the risk assessment model of gastric cancer was constructed.Results:(1)In univariate analysis,there were significant differences in age,sex,education level,total annual income of the whole family,smoking,alcohol consumption,garlic intake,whole grain intake,hot food intake,Helicobacter pylori infection,atrophic gastritis,marital life and interpersonal relationship between the case group and the control group(P < 0.05).Multivariate logistic regression analysis showed that age(OR=1.072,95%CI=1.057-1.087),daily consumption of garlic(OR=1.553,95%CI=1.055-2.287)and frequent consumption of garlic(OR=1.647,95%CI=1.180-2.299)were risk factors for gastric cancer,while secondary education(OR=0.705,95%CI=0.535-0.929)was protective factors for gastric cancer.In the stratified analysis of Helicobacter pylori infection status,garlic intake was associated with gastric cancer,and frequent consumption of garlic was a risk factor for gastric cancer(OR=2.349,95%CI=1.105-4.997,P<0.05).In the stratified analysis of the eradication status of Helicobacter pylori,garlic intake was associated with gastric cancer.Daily consumption of garlic(OR=1.593,95%CI=1.007-2.521,P < 0.05)and regular consumption of garlic(OR=1.583,95%CI=1.065-2.351,P<0.05)were risk factors for gastric cancer.(2)The gastric cancer risk index model is established based on the Harvard cancer risk index model.The model can effectively quantify the risk of gastric cancer and has good prediction efficiency.Conclusion: In the state of Helicobacter pylori infection and eradication,garlic intake is a risk factor for gastric cancer.In this study,a gastric cancer risk assessment model is constructed and verified according to the risk factors of gastric cancer in the results of research.The model can effectively quantify the risk of gastric cancer and has good prediction efficiency.
