Objective:To understand the current situation of team conflict and team cohesion of clinical key specialties during the reform of public hospitals,and to explore the mechanism of team conflict and team cohesion on the ability of clinical key specialties,so as to provide theoretical guidance for improving the ability of clinical key specialties.Methods:1.Literature research methodWith the help of Chinese databases(CNKI,VIP,Wan fang,etc.)and English databases(Pub med,Web of Science,etc.),"team conflict,team cohesion,Team performance,key clinical specialties,team conflict,team cohesion,team.At the same time,official websites such as the National Health Commission and news websites were searched to collect and sort out relevant literature and public policies related to key clinical specialties.2.Interview methodSemi-structured interview was used to interview 6 leaders of key clinical specialties according to the designed interview questions,in order to deepen the understanding of the current situation of team conflict and team cohesion of key clinical specialties.3.Questionnaire survey methodTaking provincial and municipal key clinical specialist teams as the research object,the questionnaire was designed according to the position characteristics of clinicians,including the basic situation questionnaire of team members,team cohesion scale,and team conflict scale.4.Statistical analysisspss26.0 was used for data analysis.Descriptive statistics and analysis of variance were used to understand the population status and team difference characteristics of team conflict,team cohesion and clinical key specialty competence.Pearson correlation analysis was used to study the relationship between team conflict,team cohesion and clinical key specialty competence.Hierarchical regression analysis was used to test the main and moderate effects of team conflict,team cohesion and clinical key specialty ability.Results:1.The average value of team size was(201.580±381.465),the average value of construction investment was(1985.352±3470.954),the average value of team conflict was(22.57±3.00),and the average value of team cohesion was(43.22±1.86).The average of clinical key specialty competence was(45.518±35.527).2.There were significant differences in team type in team conflict and clinical key specialty ability,significant differences in team size in team conflict and team cohesion,and significant differences in construction funds in team conflict,team cohesion and clinical key specialty ability.3.Pearson correlation analysis showed that there was a significant negative correlation between team conflict and clinical key specialty ability,and a significant positive correlation between team cohesion and clinical key specialty ability.4.Hierarchical regression analysis showed that there was an inverted U-shaped relationship between team conflict and clinical key specialty ability;The relationship between team cohesion and the ability of key clinical specialties was positive and significant(β=1.80,P < 0.05),that is,there was a significant positive correlation between them.Team cohesion played a moderating role between team conflict and the ability of key clinical specialties.The higher the team cohesion,the weaker the inverted U-shaped relationship between team conflict and the ability of key clinical specialties.Conclusions:1.In clinical key specialty teams,team conflict can lead to an inverted U-shaped change in their ability level.Therefore,we should pay attention to the "too good effect" of conflict and adjust the level of team conflict in time.2.Among the key clinical professional teams,the technical level and medical quality of the key clinical professional teams with low team cohesion were worse than those with high team cohesion.Therefore,improving team cohesion is conducive to alleviating the adverse impact of excessive conflict on the ability of key clinical specialties and enhancing the ability of key clinical specialties construction...
