Objective:1.To explore the status quo of discharge readiness of COPD patients and explore its influencing factors;2.Based on the Behavior Change Wheel(BCW)theory,constructed an intervention plan for discharge readiness of COPD patients,and evaluated its effect.Methods:The study is divided into three parts:1.Current survey: 156 patients with COPD in a tertiary hospital in Shanxi Province were surveyed using the General Information Questionnaire,Discharge Readiness Scale,Self-Efficacy Scale,and Discharge Guidance Quality Scale,and the factors influencing discharge readiness were explored using univariate and multiple linear regression analyses.2.Protocol construction: a BCW theory-based intervention protocol for COPD patients’ discharge preparation was constructed through literature review,expert meetings and pre-experiments.3.Program implementation: a class experimental study was conducted,and 69 COPD patients were selected by convenience sampling from July to September 2022 in a tertiary hospital in Shanxi province,and were divided into control and intervention groups according to the time of admission;35 in the control group implemented face-to-face verbal preaching,scheduled follow-up and other conventional care,while34 in the intervention group implemented a discharge readiness intervention program for COPD patients based on BCW theory;the two groups were compared in terms of discharge The readiness,quality of discharge instructions,self-efficacy,and CAT scores of patients in the two groups were compared.Results:1.Survey of the current situation(1)COPD patients had a discharge readiness score of 81.13 ± 11.33,a quality of discharge instructions score of 183.19±10.52,and a self-efficacy score of 86.72±11.87,all of which were at a low level;regression analysis showed that disease duration,number of comorbidities,quality of discharge instructions,and self-efficacy were factors influencing their discharge readiness.2.Program constructionTo construct a discharge readiness intervention program for COPD patients,including three intervention elements of competence,motivation,and opportunity;education,training,persuasion,motivation,and modeling,to achieve six intervention functions.3.Empirical findingsA total of 69 study subjects were included in this study,34 cases in the intervention group and 35 cases in the control group.The readiness for discharge and the quality of discharge instructions were higher in the intervention group than in the control group,with statistically significant differences(P<0.05);the self-efficacy and CAT scores were higher in the intervention group than in the control group at discharge and at the 1st and 3rd months after the intervention,with statistically significant differences(P<0.05);the rate of unplanned readmission within 3 months after discharge was 8.6% in the intervention group,compared with 11.76% in the control group.Conclusion:(1)COPD patients were moderately ready to be discharged from the hospital,and the influencing factors were disease duration,number of comorbidities,quality of discharge instructions,and self-efficacy.(2)Intervention programs based on BCW theory can improve COPD patients’ readiness for discharge,the quality of discharge instructions,improve COPD patients’ self-efficacy and health-related quality of life.
