Objective:To investigate the basic data,blood pressure level,blood pressure variability,blood pressure rhythm,biochemical index,sleep monitoring index of patients with hypertension complicated with obstructive sleep apnea hypopnea syndrome(OSAHS),and analyze the risk factors of hypertension complicated with OSAHS.Early and reasonable intervention for patients with hypertension and OSAHS can prevent the occurrence of cardiovascular and cerebrovascular diseases,and at the same time provide new ideas and basis for the formulation of clinical antihypertensive treatment regimens for hypertension.Methods:From June 2020 to June 2022,200 patients over 18 years old with hypertension who were hospitalized in the Department of Cardiology of Shanxi Bethune Hospital from June2020 to June 2022 were continuously collected,and polysomnography was monitored due to symptoms of snoring,daytime sleepiness,and possible apnea during sleep.Collect the basic information of the selected patients such as age,gender,personal history,etc.,general conditions such as: height,weight,blood pressure,biochemical indicators include: blood sugar,blood lipids,liver and kidney function,homocysteine,and 24 h ambulatory blood pressure value,sleep monitoring indicators.According to the AHI,the study subjects were divided into four groups,namely simple hypertension,hypertension with mild OSAHS,hypertension with moderate OSAHS,hypertension with severe OSAHS.The study data were statistically analyzed,and the differences between the four groups in basic data,biochemistry,sleep,blood pressure and other indicators were compared,and the risk factors of hypertension combined with OSAHS were analyzed.Results:1.There were no statistical differences between groups in sex,smoking history,alcohol history,family history of hypertension,height,weight,SBP,DBP,and course of hypertension(P>0.05).Age and BMI values were higher in the hypertensive group and mild,moderate,and severe OSAHS than in the simple hypertension group,and the difference was statistically significant(P<0.05).2.There were no significant differences between the groups in the values of 24 h SBP,24 h DBP,d SBP,d DBP,n SBP and n DBP(P>0.05).3.The values of hypertension combined with mild,moderate and severe OSAHS were higher in the group with 24 h SBPV,24 h DBPV,d SBPV,d DBPV,n SBPV and n DBPV than in the group with simple hypertension.There were significant differences in the values of24 h SBPV,24 h DBPV,d SBPV,d DBPV,n SBPV and n DBPV between the groups with mild,moderate and severe OSAHS.The proportion of nonscoop-type blood pressure rhythms in the group with mild,moderate and severe OSAHS was higher than that in the group with simple hypertension(P<0.05).4.There were significant differences between groups in AHI,ODI,mean Sp O2,lowest Sp O2,CT90%,and maximum pause time(P<0.05).5.There were significant differences in FBG,TC,LDL-C,HDL-C,AST,HCY,and UA values between the groups(P<0.05),and there was no significant difference in TG,ALT,Cr and BUN values(P>0.05).6.Pearson correlation analysis was performed on biochemical indexes,blood pressure variability and blood pressure rhythm between groups with hypertension and different degrees of OSAHS,and the results showed that AHI was correlated with BMI,FBG,TG,LDL-C,HCY,UA and all were positively correlated(P<0.05).7.Taking whether hypertension combined with OSAHS was the dependent variable,the four groups were compared with different indicators: age,BMI,FBG,TG,TC,LDL-C,HDL-C,AST,HCY,UA as independent variables for binary logistic regression analysis,and the results showed that age,BMI,FBG,and HCY were risk factors for hypertension combined with OSAHS(P<0.05).Conclusion:1.The proportion of age,BMI,blood pressure variability and non-spoon-type blood pressure rhythm in the group with hypertension and different degrees of OSAHS was higher than that in the group with simple hypertension,and increased with the increase of disease severity.2.Sleep apnea and hypoxia in the group with hypertension and different degrees of OSAHS were more severe than those in the simple hypertension group,and worsened with the increase of the severity of the disease.3.The biochemical indexes such as FBG,TC,LDL-C,HDL-C,AST,HCY,UA and other biochemical indexes in the group with hypertension and different degrees of OSAHS were higher than those in the group with hypertension alone.4.In the group with hypertension and different degrees of OSAHS,AHI was associated with BMI,FBG,TG,LDL-C,HCY,and UA.5.Age,BMI,FBG,and HCY are risk factors for hypertension combined with OSAHS. |
