Objective:To investigate the effects of ciprofol and propofol on hemodynamics in type 2diabetes patients during anesthesia induction.Methods:From June 2021 to November 2022,sixty patients with T2DM scheduled for elective surgery in the First Hospital of Shanxi Medical University were randomly divided into two groups:ciprofol group(n=30)and propofol group(n=30).Patients in ciprofol group received sufentanil 0.3ug/kg,ciprofol 0.3-0.4mg/kg and cis-atracurium0.15mg/kg.Patients in propofol group received sufentanil 0.3ug/kg,propofol 1.5-2.0mg/kg and cis-atracurium 0.15mg/kg.The changes in mean arterial pressure(MAP)and heart rate(HR)were observed before induction of anesthesia(T0),1min before endotracheal intubation(T1),1min after endotracheal intubation(T2),3min after intubation(T3)and 5min after intubation(T4)of both groups.Blood samples were collected in T0、T2、T4 to compare the changes of norepinephrine,cortisol,glucose and insulin in plasma.Adverse effects such as injection pain were also noted.Results:(1)When comparing the clinical data between the two groups,there was an interaction between the time points of MAP and the groups(FInteraction=16.246,P<0.001);the difference in the main effect between the groups was significant(Fgroup=4.354,P=0.041).At T1 and T2 time points,MAP in ciprofol group was higher than that in propofol group(P<0.05).(2)The HR level after induction in ciprofol group decreased less than that in propofol group before surgery(P<0.001).(3)The norepinephrine level after intubation in ciprofol group increased more than that in propofol group(P<0.001).(4)Plasma cortisol,blood glucose and insulin concentration did not differ significantly between the two groups(P>0.05).(5)The incidences of hypotension(16.7%vs40%)and injection pain(6.7%vs36.7%)in ciprofol group were lower than those in propofol group(P<0.05).Conclusion:Compared with propofol1.5-2.0 mg/kg,ciprofol0.3-0.4 mg/kg showed more stable hemodynamics for anesthesia induction of type 2 diabetes patients.The mechanism may be related to higher change of plasma concentration of norepinephrine after intubation and lower incidences of injection pain.In terms of the effect on the body’s perioperative stress response,the two basically do not differ. |
