Objective:This study aims to explore the effects of nutrition management based on PDCA cycle on the nutritional status of patients with gastric cancer undergoing postoperative chemotherapy,in order to help patients complete postoperative diet reconstruction as soon as possible,promote their healthy eating behavior,improve their nutritional status,quality of life and disease prognosis.At the same time,it can provide references for expanding the application of PDCA cycle management model in clinical practice.Methods:This study is a quasi-experimental study.Convenient to select 90 patients with gastric cancer who underwent the first three chemotherapy treatments in the oncology departments of a tertiary hospital in Shanxi Province from March 2022 to November2022 as the research objects.Patients were divided into two groups according to their respective wards at admission: ward 1 was the control group and ward 2 was the intervention group,with 45 cases in each group.Intervention was performed in the first,second and third chemotherapy cycles(each chemotherapy cycle was 21 days).Patients in the control group were given conventional nutrition management,and the intervention group received nutrition management based on PDCA cycle on the basis of the control group.Dietary compliance rate,nutritional support compliance rate,PG-SGA score,BMI,total protein,albumin,serum prealbumin and EORTC QLQ-C30 score of the two groups were compared before intervention and after the first,second and third cycles of chemotherapy.Results:1.During the study,3 patients in the control group were interrupted due to severe adverse reactions of chemotherapy,and 42 patients were finally effective;In the intervention group,2 cases were interrupted due to severe adverse reactions of chemotherapy,1 case died due to deterioration of the condition,and 42 cases were finally effective.A total of 84 effective subjects were studied.2.There were no statistically significant differences in general information and PG-SGA score,BMI,total protein,albumin,serum prealbumin and EORTC QLQ-C30 score between the two groups before intervention(P>0.05),indicating that the two groups were comparable.3.After intervention,the dietary compliance rate and the compliance rate of nutritional support in the intervention group were better than those in the control group,with statistical significance(P<0.05);After the first chemotherapy cycle,the PG-SGA score,BMI,total protein,albumin and serum prealbumin of all periods in the intervention group were better than those in the control group,with statistical significance(P<0.05).The results of repeated measurement ANOVA showed that,PG-SGA score,BMI,total protein,albumin and serum prealbumin in two groups had temporal,intergroup and interaction effects(P<0.05).4.After the intervention,the overall health status and scores of body,emotion,cognition,role and social function of the intervention group were higher than those of the control group,with statistical significance(P<0.05),while the scores of nausea and vomiting,loss of appetite,constipation,diarrhea,fatigue and insomnia were lower than those of the control group,with statistical significance(P<0.05).However,there was no significant difference in pain,shortness of breath and economic difficulty scores between the two groups(P>0.05).Conclusion:Nutrition management based on PDCA cycle can improve the dietary compliance of patients with postoperative chemotherapy for gastric cancer,promote the establishment of healthy eating behaviors,and then achieve the management goals of improving nutritional status and quality of life,which is worthy of further practice and promotion in patients with postoperative chemotherapy for gastric cancer and other diseases.
