Objective:The purpose of this study was to explore the predictive value of soluble growth stimulating expression gene 2 protein(sST2),N-terminal B-type natriuretic peptidogen(NTpro BNP),and neutrophil-to-lymphocyte ratio(NLR)on the 6-month prognosis of patients with acutely decompensated chronic heart failure,comparing the predictive value of the three combined and single index tests.Methods:This study is a retrospective study.A total of 125 inpatients who were diagnosed with acutely decompensated chronic heart failure in the Second Hospital of Shanxi Medical University from October 2021 to June 2022 were selected.Venous blood was collected from these patients within 24 hours of admission and sent uniformly to the laboratory for determination of serum soluble sST2,N-terminal brain natriuretic peptide precursor,and neutrophil lymphocyte ratio.All patients were followed up for 6 months with a composite endpoint of cardiac death and rehospitalization due to heart failure.17 patients were lost in follow-up and 108 patients completed follow-up.The patients who completed the follow-up were divided into a non endpoint event group and an endpoint event group based on whether an endpoint event occurred.The non endpoint event group consisted of 49 patients,34 males,and 15 females,aged 63.5 ±12.5 years.The endpoint event group consisted of 59 patients,including 34 males and 25 females,aged 72.64 ± 9.96 years.Statistical analysis was performed at the end.Results:(1)The age,sST2,NLR,NT-pro BNP,myoglobin,creatine kinase isoenzyme,and cardiac output levels were statistically higher in the endpoint event group compared with the non-endpoint event group(P < 0.05),and body mass index levels were statistically lower in the endpoint event group compared with the non-endpoint event group(P < 0.05).(2)Binary logistic regression showed that sST2,NTpro BNP,NLR,and age were all predictors of prognosis in patients with acutely decompensated chronic heart failure.The area under the curve(AUC)of sST2,NT-pro BNP,NLR,and the combination of the three was found to be 0.771,0.697,0.677,and 0.848,respectively,by drawing the subject operating characteristic curve(ROC curve).this result suggests that all three indicators have predictive value for the prognosis of acutely decompensated chronic heart failure patients.The Delong test revealed a statistically significant difference between the AUC of the three combined tests and the AUC of the single test of each index(P <0.05).A two-by-two comparison of the three indicators revealed no statistically significant difference in AUC between single indicator tests(P >0.05).Conclusion:ST2,NLR,and NT-pro BNP all have predictive value for the prognosis of patients with acute exacerbation of chronic heart failure within 6 months.The higher levels of the above indicators at admission may be related to the poor prognosis of patients with acutely decompensated chronic heart failure.Significantly better than the predictive value of each single index detection.
