Objective:The purpose of this study is to investigate the differences in serum miR-206 expression between Traumatic Brain Injury(TBI)patients and healthy people,and analyze the correlation between miR-206 and other protein markers,to provide a new auxiliary examination indicator for the diagnosis and prognosis ofTBI.Methods:1.Select 30TBI patients who were treated in the Department of Neurosurgery and Emergency Medicine of the First Hospital of Shanxi Medical University from January2022 to December 2022.According to Glasgow coma score,patients were divided into mild or moderateTBI(mTBI)group(8 <GCS<15)and severeTBI(sTBI)group(GCS≤8).The control group was selected as 15 healthy persons who visited the health examination center of the First Hospital of Shanxi Medical University during the same period.8-10 ml of peripheral blood was collected fromTBI patients within 24 hours before operation,and 8-10 ml of peripheral blood samples from physical examinees in the control group were allowed to stand for 2 hours,and the serum was extracted by centrifugation and stored at-80°C.2.Serum exosomes were extracted by ultracentrifugation,and the size,quality,and concentration of exosomes were identified by Nanoparticle Tracking Analysis(NTA),Transmission Electron Microscopy(TEM),and Western Blot(WB).Using miR-206 primers to reverse transcribe miR-206,and then detect the expression level of miR-206 in each sample by Quantitative Real-time PCR(q PCR).The difference in the expression of miR-206 in the three groups of samples was analyzed,and the receiver operating characteristic(ROC)curve was constructed by using SPSS software to evaluate the diagnostic efficacy of miR-206 forTBI.3.The Glasgow Outcome Scale(GOS)of patients in the sTBI group was followed up at 6 months after injury.To analyze the difference of serum miR-206 expression between patients with poor prognosis and those with good prognosis,and the correlation analysis between the expression level of serum miR-206 and GOS was performed to detect the ability of miR-206 to predict the prognosis of sTBI patients.4.The ELISA method was used to detect the concentration of Brain-berived Neurotrophic Factor(BDNF),Neuron-specific Enolase(NSE),Recombinant S100 Calcium Binding Protein B(S100B)in the serum ofTBI patients enrolled in the group,and the relationship between the concentration of BDNF,S100 B,NSE and the expression of miR-206 was analyzed.Results:1.The relative expression level of serum miR-206 in mTBI group was 2.83776(5.47544),that in sTBI group was 5.35575(24.45846),and that in healthy people was1.06816(1.17163).The expression of serum miR-206 in mTBI group and sTBI group was significantly higher than that in healthy people,and the difference was statistically significant.There was no significant difference in serum miR-206 expression between sTBI group and mTBI group.2.The ROC curve showed that the area under the curve(AUC)of serum miR-206 in diagnosingTBI was 0.871,and when 1.71253 was taken as the cut-off value for diagnosingTBI,the sensitivity and specificity were 86.7% and 80%,respectively.3.In sTBI patients,the expression of serum miR-206 in patients with poor prognosis was significantly higher than that in patients with good prognosis.Spearman correlation analysis showed that the expression of serum miR-206 in patients with sTBI was significantly negatively correlated with GOS(r=-0.163,P=0.015).4.The concentration of BDNF in the serum ofTBI patients was positively correlated with the expression of miR-206(r=0.425,P=0.019),and the concentration of NSE was negatively correlated with the expression of miR-206(r=-0.464,P=0.01);There was no correlation between the concentration of S100 B and the expression of miR-206(r=-0.268,P=0.152).Conclusion:1.In this experiment,the expression of serum miR-206 in the mTBI group and the serum miR-206 in the sTBI group was significantly higher than that in the healthy group,and the difference was statistically significant;There was no significant difference in serum miR-206 expression between sTBI group and mTBI group.2.Serum miR-206 can be used as a novel biomarker ofTBI for clinical diagnosis,but it cannot predict the severity ofTBI.3.Serum miR-206 may serve as a novel biomarker to predict the prognosis of sTBI patients.4.Serum miR-206 expression inTBI patients was positively correlated with BDNF concentration,and negatively correlated with NSE concentration;Serum miR-206 expression was not correlated with S100 B concentration.
