Purpose:To compare the clinical effects of customized coral hydroxyapatite bone block and autogenous bone block for severe alveolar bone defects,and to evaluate the safety and efficacy of coral hydroxyapatite bone blocks.Methods:Thirty-nine patients with severe alveolar bone defect were recruited from Oct2018 to Jan 2021 in the Affiliated Hospital of Qingdao University.Eighteen patients underwent customized coral hydroxyapatite bone blocks grafting(experimental group),twenty-one patients underwent autogenous bone blocks grafting(control group).To compare the bone augmentation and resorption between the two groups,took CBCT before(T0),immediately after(T1),six months after augmentation surgery(T2)and immediately after implant surgery(T3).Measured the bone width of 0mm(L1),3 mm(L2),and 6 mm(L3)from the top of the alveolar ridge,three times by the same physician.Recorded the pain intensity,satisfaction and complications(soft tissue dehiscence,infection or bone necrosis)after bone grafting.SPSS 26.0 software was used for statistical analysis,and the counting data was represented( ±s).If the data test conforms to a normal distribution,the independent sample t-test is used for inter group comparison;Otherwise,the inter group comparison is performed using χ2 Inspection.There was significant difference(P <0.05).Results:All the 39 patients were implanted successfully after bone grafting and finished restorations.The average follow-up was(17.28±4.34)months.Bone mass in both groups was significantly increased immediately after bone grafting.Patients in both groups had different degrees of bone formation and bone mass absorption 6 months after bone grafting.There was no significant difference in bone augmentation and bone absorption 6 months after operation between the two groups(P>0.05).The pain response of CHA bone block group was significantly different than that of autologous bone block group(P<0.05).Both groups of patients were relatively satisfied with the clinical satisfaction survey(P>0.05).Conclusion:1.Customized coral hydroxyapatite bone blocks is safe and effective in using for bone augmentation,with shorter surgical time,lighter postoperative reaction,more stability,and easier to be accepted by patients.2.At present,digital bone transplantation is still in its initial stage.This study has achieved good results in using CAD/CAM to create personalized grafts.It can be foreseen that with the deepening of research,digital and personalized grafts will be widely used in clinical practice.3.Customized coral hydroxyapatite bone block has great potential.At present,there are two problems to be solved,one is that it only has bone conductivity,the other is that it is fragile.Improving its biological and mechanical properties through tissue engineering is a future research hotspot.
