Objective:To investigate the relationship between the expression of serum exosomal mi R-29 C and postoperative cognitive dysfunction in elderly patients undergoing surgery.Methods:119 elderly patients undergoing elective surgery in our hospital were selected.The selected patients were scored with MOCA scale(simple intelligence state examination)on the first day before operation and on the first,seventh and twenty-first days after operation.The selected patients were divided into POCD group(51 cases)and non POCD group(68 cases)according to the score of MOCA scale on the first day after operation.S100-β Protein,NSE concentration and serum exosomal mi R-29 C expression level were detected and analyzed on the first day before operation and the first day after operation.Results:1)the MOCA score of POCD group was significantly lower than that of the first day before operation(P < 0.05),but the MOCA score of non POCD group was not significantly changed(P > 0.05).2)Two groups of patients at S100-β The concentration of protein,NSE and the expression level of serum exosomal mi R-29 C were significantly higher than that on the first day before operation(P < 0.05),and S100-β The concentration of protein,NSE and the expression level of serum exosomal mi R-29 C were significantly higher than those of non POCD patients on the 1st day after operation(P < 0.05).3)There was a negative correlation between MOCA score and the expression level of mi R-29 C in serum exosomes in POCD group(P < 0.05).The ROC curve analysis of the subjects showed that the NSE and S100-β The AUC of the expression level of protein and exosomal mi R-29 C to predict the risk of cognitive dysfunction in elderly patients after surgery were 0.891,0.908 and0.918,respectively.Conclusion:The occurrence of POCD in elderly patients is related to the increase of the expression level of serum exosomal mi R-29 C,and the expression level of serum exosomal mi R-29 C is negatively correlated with the score of MOCA scale.Early monitoring of POCD can provide basis for the occurrence and development of POCD in elderly patients after surgery,the judgment of disease condition and clinical intervention. |
