Objective:Acute kidney injury(AKI)is a common disease in the intensive care unit(ICU).In this retrospective study,we aimed to investigate the incidence,etiology and influencing factors of AKI in ICU patients,which may further provide clinical evidence for early prevention,identification and effective treatment of AKI.Methods:A single-center case-control study was conducted on 560 ICU inpatients admitted to Shanxi Baiqiuen Hospital from January to June 2021,and a database was created using SPSS25.0 software.Patient information,basic diseases,laboratory results,potential risk factors and hospitalization days in ICU were recorded.Patients were divided into AKI group(174cases)and non-AKI group(386 cases)based on the occurrence of AKI.Subsequently,t-test,x2 test,univariate and multivariate logistic regression analyes were used to screen the occurrence and influencing factors of patients with AKI in ICU.Results:The incidence of AKI was 31.07% in ICU,and the male-to-female ratio of AKI cases was 2.11:1(118:56).Elderly patients were identified as high-risk individuals for AKI.The proportion of AKI caused by prerenal,renal,and postrenal factors was 59.20%,34.48%,and6.32%,respectively,in which prerenal factors were the primary causes of AKI in critically ill patients,with sepsis-induced infection and nephrotoxic drug use being the major etiologies in ICU.Multivariate logistic regression analysis revealed that the independent risk factors for AKI in critically ill patients were the number of organ failures ≥2(OR=3.486,95%CI:2.003-6.070,P<0.001),use of vasoactive drugs(OR=6.398,95%CI:2.650-15.449,P < 0.001),APACHE II score ≥20(OR=2.874,95%CI:1.886-4.379,P < 0.001)and mechanical ventilation(OR=2.198,95%CI: 1.248-3.872,P=0.006).Conclusions:In conclusion,AKI has a high incidence in critically ill patients.The number of organ failures ≥ 2,use of vasoactive drugs,APACHE II score ≥ 20 and mechanical ventilation are independent influencing factors for the occurrence of AKI in critically ill patients.Clinicians should be highly aware of the influencing factors for AKI,which helps adopt different levels of treatment to improve the prognosis of AKI.
