Objective:To detect the expression of MUC16,Vimentin and E-cadherin in salivary gland mucoepidermoid carcinoma(MEC),and analyze the relationship between them and the clinicopathological features of MEC,to explore the relationship between MUC16 and EMT,and preliminarily understand the potential pathogenesis of MEC,At the same time,the sensitive factors related to the prognosis of MEC are discussed in order to find a new therapeutic target for clinic,and then optimize the therapeutic strategy and guide the clinical precise treatment.Methods:The clinical data of MEC patients who were treated by surgery in the first Hospital of Shanxi Medical University from 2012 to 2021 were collected.Finally,58 cases of MEC patients’cancer tissues and 20 cases of corresponding paracancerous tissues were screened.The expressions of MUC16,Vimentin and E-cadherin proteins in the two groups were detected by immunohistochemical staining,To analyze the correlation between them and their influence on the prognosis of MEC.Results:(1)the positive expression rate of MUC16 protein was 63.8%(37/58),which was significantly higher than that in paracancerous tissues(10%).The positive rates of MUC16 in low,middle and high grade MEC were 47.8%(11/23),63.2%(12/19)and87.5%(14/16),respectively,and the positive expression rate of MUC16 increased with the decrease of the degree of MEC differentiation.(2)The expression of MUC16 was related to age(X~2=7.077,P=0.029),pathological grade(X~2=6.435,P=0.040),lymph node metastasis(P=0.041)and clinical stage(X~2=7.817,P=0.005),but not to sex,tumor size and location(p>0.05).(3)VImentin was not expressed in normal salivary glands,and the positive expression rate in MEC was 43.1%(25/58).The expression of Vimentin was correlated with pathological grade(X~2=8.713,P=0.013)and lymph node metastasis(P=0.050),but not with sex,age,size,tumor location and clinical stage(P>0.05).The expression of E-cadherin was highly expressed in paracancerous salivary glands(positive rate 100%),and decreased in MEC tissue,and the positive expression rate is 44.8.%(26/58).The expression of E-cadherin was correlated with clinical stage(X~2=4.416,P=0.036),pathological grade(X~2=8.322,P=0.016)and lymph node metastasis(P=0.005),but not with sex,age,size and tumor location(p>0.05).(4)In MEC,the expression of MUC16 was correlated with the expression of Vimentin(rs=0.438,P=0.001),and negatively correlated with the expression of E-cadherin(rs=-0.403,P=0.002).The expression of Vimentin was significantly negatively correlated with the expression of E-cadherin(rs=-0.435,P=0.001).(5)The results of log-rank test and Kaplan-Meier survival curve showed the expression of MUC16 protein was correlated with the prognosis of MEC(X~2=7.062,P=0.008),Compared with the patients with high expression of MUC16,patients with low expression of MUC16 MEC had longer DFS,.However,the expression of Vimentin and E-cadherin was not related to DFS in MEC patients.In addition,COX multivariate analysis showed that the expression of MUC16 protein and clinical stage were the risk factors of poor prognosis in patients with MEC.Conclusion:(1)In MEC,the expression of MUC16 and Vimentin was up-regulated,while the expression of E-cadherin was down-regulated.(2)The expression of MUC16 protein in MEC tissues is higher than that of salivary gland tissue adjacent to cancer,which is related to age,pathological grade,clinical stage,lymph node metastasis and prognosis,and the prognosis of patients with high expression of MUC16 is poor.(3)The expression of Vimentin protein is higher than that of salivary gland tissue adjacent to cancer,which is related to pathological grade and lymph node metastasis,while the expression of E-cadherin protein is lower than that of of salivary gland tissue adjacent to cance,which is related to pathological grade,clinical stage and lymph node metastasis.however,the expression of both of them did not affect the prognosis of MEC patients.(4)The expression of MUC16 was positively correlated with vimentin,negatively correlated with E-cadherin,and correlated with E-cadherin and vimentin,the combined detection of the three is helpful to evaluate the pathological grade and distant metastasis of patients with MEC.
