ObjectiveThe self-management scheme for young and middle-aged patients with coronary heart disease was established on the basis of evidence, and whether the scheme can help patients establish normative selfmanagement behavior, improve the level of self-efficacy,adjust the mood alleviate psychological pressure, improve the quality of life.Methods1.The relevant literature and guidelines at home and abroad were retrieved, and the recommendations were summarized to form a summary of the evidence. The PDCA cycle and health promotion model were used as the framework, and the self-management behavior scale for patients with coronary heart disease was referred to.After discussion by the members of the research group, the first draft was formulated. The draft was revised through two rounds of expert consultation and preliminary experiment, and the selfmanagement plan for young and middle-aged patients with coronary heart disease was finally formed.2.This study is a quasi-experimental study. The method of convenience sampling was adopted in this study to select 64 patients hospitalized in the cardiovascular department of a Class III Grade-A general hospital in Kaifeng City from March 2022 to July 2022.According to the order of admission of patients were numbered, the single number was the control group, and the double number was the intervention group until the number of the two groups was saturated, with 32 people in each group. The control group was treated with routine nursing intervention. The intervention group adopted the self-management scheme. The changes in physiological indicators,self-management ability,self-efficacy level, quality of life, anxiety and depression before and after intervention were compared between the two groups.Using t test,χ2 test and repeated measurement analysis method, with P < 0.05 for the difference was statistically significant.Results1.Through systematic search,a total of 18 articles were included, including 10 guidelines,2 expert consensus,2 systematic reviews and 4 randomized controlled trials. The included literature was extracted, translated, analyzed and summarized.A total of 49 recommendations were formed from five aspects: team construction and responsibility,purpose,evaluation,self-management plan content and effect evaluation.2.Based on the PDCA cycle and health promotion model as the framework, the self-management behavior scale for patients with coronary heart disease was referred to,and the relevant evidence was summarized. After two rounds of expert letter consultation and preliminary experiment, the selfmanagement scheme for young and middle-aged patients with coronary heart disease was formed.3.In this study,a total of 61 patients were completed, including 30 in the control group and 31 in the intervention group. The comparison of general data between the two groups showed no statistical difference(P>0.05).4.Before intervention, there was no statistically significant difference in physiological indicators between the two groups(P>0.05).After 3 months of intervention, there were statistically significant differences in the number of patients with normal blood pressure, blood lipid and BMI indicators between the two groups(P<0.05).There was no statistically significant difference between the two groups in the number of patients with normal blood glucose level(P>0.05).5.Before intervention, there was no statistical difference in self-management scores between the two groups(P>0.05).After 1 month and 3 months of intervention, the self-management score of the intervention group was higher than that of the control group, and the difference was statistically significant(P<0.05).6.Before the intervention, there was no statistical difference in self-efficacy scores between the two groups(P>0.05).After 1 month and 3 months of intervention, the self-efficacy score of the intervention group was higher than that of the control group, and the difference was statistically significant(P<0.05).7.There was no statistically significant difference in the quality of life scores between the two groups before intervention and 1 month after intervention(P>0.05).After 3 months of intervention, the quality of life score of the intervention group was higher than that of the control group, and the difference was statistically significant(P<0.05).8.Before intervention, there was no statistically significant difference in the anxiety score between the two groups(P>0.05).After 1 month and 3 months of intervention, the anxiety score of the intervention group was lower than that of the control group, and the difference was statistically significant(P<0.05).9.Before intervention and 1 month after intervention, there was no statistically significant difference in depression scores between the two groups(P>0.05).After 3 months of intervention, the depression score of the intervention group was lower than that of the control group, and the differences were statistically significant(P<0.05).ConclusionSelf-management for young and middle-aged patients with coronary heart disease are feasible and effective, which can help patients improve the ability of self-management,enhance self-efficacy,relieve mental tension, reduce anxiety and depression, prompting them to keep the positive attitude to deal with diseases,so as to improve the quality of life.
