| Objective Taking the dual-source spiral CT(DSCT),which is popular and widely used by medical personnel at present,as the main detection method,to deeply explore the correlation between the area of abdominal umbilical plane(VAT)and coronary heart disease(CHD).Methods 124 patients with suspected coronary angiography(CAG)who were hospitalized in the Department of Cardiology of Qingdao University Medical Group from June 2019 to November 2020 were selected. The CAG results of these patients were analyzed and grouped according to whether the coronary lumen stenosis≥50%,one is non-coronary heart disease group(n=60),the other is coronary heart disease group(n=64).All the patients in the group were given medical history consultation, examination and test, and the area of VAT was measured by DSCT scan of the abdomen and umbilicus. The data of age, sex, height, weight,Blood pressure,Fasting Blood Glucose(FBG),Glycosylated hemoglobin,Creatinine(Cr),Uric acid(UA),Total cholesterol(TC),Low density lipoprotein-cholestero(LDL-C),Triglycerides(TG)and High density lipoprotein-cholestero(HDL-C)were collected and Low-density lipoprotein. The basic parameters, atherogenic index of plasma(AIP),the area of VAT and Gensini score were compared between the two groups by SPSS26.0 arteriosclerosis.Multiple regression was used to analyze the relationship between AIP and clinical parameters, and Logistic regression was used to analyze the relationship between the area of VAT and the risk factors of coronary heart disease. The relationship between the area of VAT and the calcified plaque was analyzed by multivariate association analysis. The receiver operating characteristic(ROC)curves were obtained by using ggpolt2 tool kit of R language software.Result(1)The age,Cr and LDL of non-CHD group were significantly lower than those of CHD group(P<0.05).HDL of non-CHD group were significantly higher than those of CHD group(P<0.05).There were no significant differences in the indexes of UA,TC and TG between the two groups(P>0.05).(2)The area of VAT in the navel layer of the patients with CHD was significantly larger than that of the patients without non-CHD(P<0.05).(3)Multiple regression analysis showed that AIP was positively correlated with age,BMI,FBG,Hb A1 c,TC,TG,LDL-C;but negatively correlated with HDL-C;there was no correlation between UA,Cr,DBP and SBP(P>0.05).(4)The increase of VAT area was a risk factor for CHD(P<0.05).After adjusting the autocorrelation factors of VAT area(age, blood lipids, blood glucose),increased VAT area(P<0.05)is still an independent risk factor for CHD.(5)The scores of AIP and Gensini in CHD group were significantly higher than those in non-CHD group(P<0.05).There was no significant difference in other indexes between the two groups(P>0.05).(6)There was a positive association between increased area of VAT and calcified plaques.(7)The AUC of VAT area for the diagnosis of CHD was higher than that of age, smoking and diabetes mellitus. The AUC of combined diagnosis was significantly higher than that of single detection. Conclusions A study of the association between belly navel VAT area measured by DSCT and CHD showed that the larger the VAT area, the higher the risk of CHD.The larger the VAT area, the higher the incidence of arterial calcification plaque.VAT area is closely related to AIP value. |
