| ObjectiveTo investigate the clinical effect of edaravone-dexborneol on post-stroke cognitive impairment(PSCI).MethodA total of 100 patients diagnosed with acute ischemic stroke in the inpatient department of the Neurology Department of Jiaozuo City People,s hospital from July 2021 to July 2022 were collected,and 40 healthy people in the outpatient department were selected as baseline group during the same period.Montreal Cognitive Assessment(Mo CA)and Mini-mental state examination(MMSE)scores were used to divide 100 patients with acute ischemic cerebral infarction into two groups,including 30 patients in the normal cognitive function group and 70 patients in the cognitive dysfunction group.The cognitive dysfunction group was then divided into an experimental group(40 cases,conventional therapy + Edaravondexcamphorol)and a control group(30 cases,conventional therapy)based on whether or not Edaravondexcamphorol was used.Blood tests and cognitive scale assessment were performed on day 1,day 14 and day 90 after onset.The improvement of biomarkers of malondialdehyde(MDA),C-reactive protein(CRP),adiponectin(APN),interleukin-6(IL-6)and 8-hydroxy-deoxyguanylate(8-OHDG)in experimental group and control group before and after treatment was observed by enzymed-linked immunosorbent assay(ELISA).MMSE and MOCA neuropsychological assessment scale were used to evaluate the improvement of cognitive function in the experimental group and the control group before and after treatment.Results(1)There were no significant differences in age,smoking,sex,alcohol consumption,hypertension and coronary heart disease between the acute ischemic stroke group and the baseline group,P > 0.05.(2)On the first day of onset,compared with the baseline group,the levels of 8-hydroxy-deoxyguanylate(8-OHDG),interleukin-6(IL-6),malondialdehyde(MDA)and C-reactive protein(CRP)in the right cognitive function group and the cognitive disorder group were significantly increased,and the levels of adiponectin were decreased,with statistical difference(P < 0.05);However,there were no significant differences in IL-6,CRP,MDA,8-OHDG and adiponectin between normal cognitive function group and cognitive dysfunction group(P > 0.05).On the 14 th and 90 th day,compared with the baseline group,the levels of 8-hydroxy-deoxyguanylate(8-OHDG),interleukin-6(IL-6),malondialdehyde(MDA)and Creactive protein(CRP)in the cognitive dysfunction group and the normal cognitive function group were increased,and the levels of adiponectin were decreased,with statistical difference(P < 0.05).However,the serum levels of IL-6,CRP,MDA and 8-OHDG in the cognitive dysfunction group were remarkablely lower than those in the cognitive normal group,and the level of adiponectin was remarkablely higher than that in the cognitive normal group,with statistical difference between the two groups(P < 0.05).(3)There were no significant differences in the levels of inflammatory factors and oxidative stress markers between the experimental group and the control group on day 1,P > 0.05;The levels of APN in the experimental group were significantly higher than those in the control group at the 14 th and 90 th days after edaravone dextrophornol treatment,while the levels of CRP,IL-6,malondialdehyde(MDA)and 8-hydroxydeoxyguanylate(8-OHDG)were lower than those in the control group,with statistical differences(P < 0.05).(4)There was no significant difference in the scores of MMSE and MOCA between the experimental group and the control group on day 1(P > 0.05).The MMSE and MOCA scores of the control group were lower than those of the experimental group on the 14 th and 90 th days after edaravone dexcamphorol treatment,with statistical significance(P < 0.05).(5)After Edaravondexcamphorol treatment,the effective rate of the laboratorial group was 85%,and that of the control group was 40%,which was remarkablely higher in the experimental group than in the control group,with statistical significance(P < 0.05).(6)Pearson correlation analysis showed that adiponectin was weakly and positively correlated with MMSE and MOCA scores.IL-6,CRP and 8-OHDG were negatively correlated with MMSE and MOCA scores.MDA was negatively correlated with MMSE and MOCA scores.(7)The results of Pearson correlation analysis showed that adiponectin was weakly negatively correlated with IL-6 and 8-OHDG.IL-6 was weakly and positively correlated with CRP.The correlation between CRP and MDA was weak and positive.MDA was weakly and positively correlated with 8-OHDG.(8)Logistic regression analysis showed that compared with the normal baseline group,adiponectin,IL-6,CRP,MDA,8-OHDG were correlated with PSCI in patients with cognitive impairment and normal cognition.Conclusions(1)Changes in the concentrations of adiponectin,CRP,IL-6,MDA and 8-OHDG biomarkers are associated with the occurrence of cognitive dysfunction after stroke.(2)Edaravone dextrocamphenol is effective in patients with cognitive impairment after stroke.(3)Edaravone dextrocamphorol can inhibit inflammatory response and oxidative stress. |
