Objective: MTr Ps(Myofascial trigger points)acupuncture combined with heat-s ensitive moxibustion was used to treat CPID(Chronic pelvic inflammat ory disease)with Qi stagnation and blood stasis. The changes in TCM symptom, relief of chronic pelvic pain and improvement in the overall quality of well-being were observed in the three groups before and after treatment in order to find more effective treatment measures fo r CPID with Qi stagnation and blood stasis.Method: A total of 66 CPID patients with Qi stagnation and blood stasis were collected from the acupuncture and moxibustion department and gynecology department of Jiangxi Provincial Hospital of Traditional Chinese Medicine, and randomized into three groups-treated group, control group Ⅰ and control group Ⅱ,in which the treatment group used MTr Ps acupuncture combined with heat-sensitive moxibustion therapy, the control group Ⅰ used conventional acupuncture with heat-sensitive moxibustion therapy, and the control group Ⅱ used MTr Ps acupuncture and moxa box moxibustion therapy. The three groups were treated for three courses,10 days for one course.Traditional Chinese Medical syndrome score,Mc Gill pain questionnaire score, and quality of life scale(WHOQOL-BREF)before and after treatment were observed in the three groups.Finally,SPSS25.0 was used for statistical analysis of the final data, and the results were discussed.Result: 1.Sixty-six patients were collected,of which 62 were completed and 4 were shed(1 case each in the treatment group and control Ⅱ group,2 cases in the control Ⅰ group),21 cases in the treatment group,20 cases in the control Ⅰ group, and 21 cases in the control Ⅱ group in the final result analysis. In the comparison of general data, the comparison of age, course of disease, pregnancy, childbirth and number of pregnancy of the three showed that they were comparable(P>0.05).In the comparison of basic disease conditions prior to treatment, the Quality of Survival Scale(WHOQOL-BREF)scores,TCM evidence scores and Mc Gill Pain Questionnaire scores showed P>0.05 and were comparable.2.In terms of TCM symptom scoring Intra-group comparison: the paired t-test was used to compare and analyze the TCM syndrome scores of the three groups before and after treatment, and the results showed that the difference was statistically significant(P<0.05),and the scores after treatment were lower than those before treatment, indicating that the TCM syndrome of patients in the treatment group, control group Ⅰ and control group Ⅱ were all relieved after treatment, and the therapeutic effect was satisfactory in improving the TCM syndrome of CPID patients with Qi stagnation and blood stasis type.Inter-group comparison: multiple local rank sum test was used to compare the TCM syndrome scores of the three groups after treatment, and the test results showed that H=19.330,P<0.05,and the difference was statistically significant. In addition, after a two-by-two comparison of the three groups,P<0.05 was found between the treatment group and the control group Ⅰ and the control group Ⅱ,and the TCM syndrome score of the treatment group was significantly different from that of the other two groups, and P>0.05 was found between the control group Ⅰ and the control group Ⅱ.There was no clear difference in the TCM symptom scores between the two control groups. The results showed that the TCM symptom scores of the treatment group were remarkably lower than those of the other two groups, and the effect in improving the TCM symptom The results showed that the treatment group had significantly lower TCM symptoms scores than the other two groups, and had better results in improving the TCM symptoms.3.Simplified Mc Gill Pain Questionnaire Intra-group comparison: the pain scores of the three groups with Qi stagnation and blood stasis type CPID in pre-and post-treatment were significantly different by the signed rank sum test,P<0.05,and the PRI,VAS,and PPI of the three groups were all improved compared with those before treatment. This shows that the three treatment groups have significant effects on emotional and sensory relief of chronic pelvic pain in patients with CPID with Qi stagnation and blood stasis. All three treatment groups had significant effects on the emotional and sensory relief of chronic pelvic pain in CPID patients with Qi stagnation and blood stasis.Inter-group comparison: multiple local rank sum test was performed for pain scores of CPID patients with Qi stagnation and blood stasis after treatment. There were obvious differences in PRI and VAS scores among the three groups after treatment(P<0.05),but no significant differences in PPI scores among the three groups(P>0.05).There was no significant difference between control group Ⅰ and control group Ⅱ(P>0.05),but there was marked difference between treatment group and control group Ⅰ and control group Ⅱ(P>0.05),and the PRI and VAS of treatment group were lower than those of control group Ⅰ and control group Ⅱ.The results showed that MTr Ps acupuncture and heat sensitive moxibustion therapy was more effective than conventional acupuncture and heat sensitive moxibustion therapy and MTr Ps acupuncture and box moxibustion therapy in improving chronic pelvic pain.4.The score of the quality of life scale Intra-group comparison: after treatment, control group Ⅰ and control group Ⅱ,compared with before treatment, scores in physiological field and social relation field were increased, while scores in psychological field and environmental field were not significantly changed. After treatment, the scores of the three groups were analyzed by multiple local rank sum test, and the data of the three groups were significantly different(P<0.05).Inter-group comparison: The rank sum test was performed for the QOL scores of the three groups of CPID patients before treatment. After treatment, there were significant differences in the physiological field(P<0.05),but no significant differences in the psychological field, social relationship field and environmental field(P>0.05).After multiple comparisons, the scores of the treatment group treated with MTr Ps acupuncture plus heat-sensitive moxibustion were more significantly higher in the field of physiology and social relations than those of control group Ⅰ and control group Ⅱ(P<0.05,the difference was statistically significant),while there was no clear difference in the field of psychology and environment compared with the other two groups(P>0.05).These results indicated that in terms of improving the quality of life of CPID patients with Qi stagnation and blood stasis, the three groups of therapy had certain improvements in their physiological fields and social relations, among which MTr Ps acupuncture plus heat-sensitive moxibustion therapy was more effective than the other two groups.5.Overall Overall efficacy evaluation: The efficacy statistics of the TCM evidence scores, the efficacy of the three groups were analyzed by multiple sample rank sum test. The test analysis showed that there was remarkable difference between the three groups(P<0.05);the comparison between the three groups showed that the difference in efficacy between the treatment group and the control group Ⅱ was more noticeable(P<0.05),and there was no significant difference in efficacy between the treatment group and control group I and control group I and control group Ⅱ(P>0.05),and the total effective rate was 95.24% in the treatment group,85.00% in control group I and 80.95% in control group Ⅱ.The overall efficacy showed that the treatment group was better than the control group Ⅱ,and the difference with the control group I was not significant. There was no great difference between the treatment group and the control group. Conclusion: Patients with CPID with Qi stagnation and blood stasis were treated with MTr Ps acupuncture combined with heat-sensitive moxibustion, and this method was able to significantly improve the symptoms and relieve chronic pelvic pain and was more effective in improving the quality of life than the other two methods. Moreover,MTr Ps acupuncture has a better effect than conventional acupuncture and heat-sensitive moxibustion in the treatment of CPID.
